摘　　要：目的建立本实验室妊娠各期特异性血清促甲状腺激素(TSH)、游离甲状腺素(FT4)的正常参考范围。方法纳入2016—2019年在南昌大学第一附属医院妇产科建卡的1521例妊娠妇女,采用化学发光免疫分析法测定妊娠早期(≤12周,T1期,n=524)、妊娠中期(13~27周,T2期,n=508)、妊娠晚期(28周至分娩,T3期,n=489)的血清TSH、FT4、游离三碘甲状腺原氨酸(FT3)、甲状腺过氧化物酶抗体(TPOAb)和甲状腺球蛋白抗体(TGAb)水平。结果T1—T3期:TSH正常参考范围分别为0.31~3.90、0.49~4.06、0.64~4.18 U·m L^(-1),FT4正常参考范围分别为12.61~20.60、11.01~19.31、10.43~20.06 pmol·L^(-1)。与T2、T3期比较,T1期TSH和FT4值差异均有统计学意义(P<0.05);T2、T3期的TSH和FT4值比较差异均无统计学意义(P>0.05)。T1—T3期TSH逐渐增高,而FT4逐渐降低(P<0.05)。结论不同妊娠期TSH、FT4参考值水平不同,应根据本地区的人群特点,建立妊娠各期特异性甲状腺激素正常参考范围。Objective To establish the normal reference ranges for serum thyrotropin(TSH)and free thyroxine(FT4)during different periods of pregnancy in our lab.Methods A total of 1521 pregnant women registered in our hospital from 2016 to 2019 were included in this study.Serum levels of TSH,FT4,free triiodothyronine(FT3),anti-thyroid peroxidase antibody(TPOAb)and anti-thyroglobulin antibody(TGAb)were measured by chemiluminescent immunoassay in the first(≤12 weeks,T1 stage,n=524),second(13-27 weeks,T2 stage,n=508),and third trimesters(28 weeks to delivery,T3 stage,n=489).Results The reference ranges of TSH and FT4 were,respectively,(0.31-3.90)U·mL^(-1) and(12.61-20.60)pmol·L^(-1) in stage T1,(0.49-4.06)U·mL^(-1) and(11.01-19.31)pmol·L^(-1) in stage T2,and(0.64-4.18)U·mL^(-1) and(10.43-20.06)pmol·L^(-1) in stage T3.The levels of TSH and FT4 in stage T1 were obviously different from those in stage T2 or T3(P<0.05),but those in stage T2 were not significantly different from those in stage T3(P>0.05).In addition,TSH levels increased but FT4 levels decreased from T1 to T3(P<0.05).Conclusion The reference ranges of TSH and FT4 are various in different pregnancy periods.The normal reference ranges for thyroid hormone during pregnancy should be established according to the characteristics of the population in this region.
