摘  要:目的探讨持续呼吸道正压通气(continuous positive airway pressure,CPAP)模式在极低出生体重早产儿转运中的应用价值。方法回顾性分析103例极低出生体重早产儿,根据是否使用CPAP转运分为CPAP前组(n=50)和CPAP后组(n=53),分析2组在入院后72 h内气管插管率、氧疗时间、肺泡表面活性物质(pulmonary surfactant,PS)使用情况和并发症发生率方面的差异。结果CPAP后组入院72 h内气管插管率(11.3%)低于CPAP前组(28.0%),多次(2~3次)使用PS治疗概率(0.0%)低于CPAP前组(8.0%)(均P<0.05);2组氧疗时间、PS使用及并发症发生率比较差异无统计学意义(P>0.05)。结论使用CPAP转运极低出生体重早产儿可以降低气管插管率及减少PS使用剂量。Objective To explore the value of continuous positive airway pressure(CPAP)in transfer of very low birth weight(VLBW)preterm infants.Methods Data of 103 VLBW preterm infants were analyzed retrospectively.According to the use of CPAP,these infants were divided into pre-CPAP group(n=50)and post-CPAP group(n=53).Endotracheal intubation rate within 72 hours after hospitalization,duration of oxygen therapy,use of pulmonary surfactant(PS)and incidence of complications were compared between the two groups.Results The endotracheal intubation rate within 72 hours after hospitalization in post-CPAP group(11.3%)was lower than that in pre-CPAP group(28.0%)(P<0.05).Furthermore,the rate of repeated PS use(2-3 times)in post-CPAP group(0.0%)was lower than that in pre-CPAP group(8.0%)(P<0.05).There were no significant differences in duration of oxygen therapy,use of PS and incidence of complications between the two groups(P>0.05).Conclusion The application of CPAP in transfer of VLBW preterm infants can reduce endotracheal intubation rate and PS dosage.
