摘　　要：王行宽主任医师认为,痫证的病位在脑,病机以肝肾亏虚,髓海不足,脑中气血逆乱为本,风痰内生,元神受扰,脑中太极动静失衡为标。王老师治疗痫证,缓解期以补肝肾、益精气、调气血为治法,方选六味地黄汤合黄芪四物汤加减;发作期以祛风痰、衡动静、醒脑慧神为治法,方选羚角钩藤汤合礞石滚痰丸或导痰汤加减。痰蒙清窍者,加石菖蒲开窍豁痰醒神,天竺黄清热豁痰;肝风夹痰者,加白僵蚕化痰熄风止痉;肝风内动者,加天麻熄风止痉、平抑肝阳;肝阳上亢者,加石决明、珍珠母、白蒺藜平肝潜阳;心肾不交、虚烦失眠者,加柏子仁养心安神,麦冬、丹参清心安神,茯神、五味子、炙远志宁心安神;脾虚湿盛者,加白术健脾益气燥湿。Chief physician Wang Xing-kuan believes that the location of epilepsy syndrome is in the brain,and the pathogenesis is based on deficiency of liver and kidney,insufficiency of sea of marrow.Disorder of qi and blood in the brain is the primary cause,while internal wind and phlegm,disturbed original spirit and imbalance of Taiji movement and static in the brain.In the remission stage of epilepsy,Professor Wang takes nourishing liver and kidney,tonifying essence and qi,regulating qi and blood as the treatment principle,and selects Six-Ingredient Rehmannia Pill combined with Astragalus Four Substances Decoction for modification.In the attack period,he takes removing wind and phlegm,balance movement and static,arousing the brain as he treatment principle,and selects Antelope Horn and Uncaria Decoction combined with Chlorite Phlegm-Removing Pill or Phlegm-Expelling Decoction for modification.For those who have phlegm to cover their orifices,Shichangpu(Rhizoma Acori Tatarinowii)can be added to open orifices and remove phlegm to awaken their spirits,and Tianlanhuang(Concretio Silicea Bambusae)is used to clear heat and resolve phlegm.For liver wind intermingled with phlegm,Baijiangcan(Bombyx Batryticatus)can be added to resolve phlegm,eliminate wind and stop spasm.For syndrome of internal stirring of liver wind,Tianma(Rhizoma Gastrodiae)can be added to eliminate wind,stop spasm and suppress liver yang.For syndrome of ascendant hyperactivity of liver yang,Shijueming(Concha Haliotidis),Zhenzhumu(Concha Margaritiferae Usta),and Baijili(Fructus Tribuli)can be added to pacify liver and suppress yang.For insomnia due to heart-kidney non-interaction,with vexation of deficiency type,Baiziren(Semen Platycladi)can be added to nourish the heart and calm the mind;Maidong(Radix Ophiopogonis)and Danshen(Radix Salviae Miltiorrhizae)can be added to clear heart fire and calm the mind;Fushen(Sclerotium Poriae Pararadicis),Wuweizi(Fructus Schisandrae Chinensis)and honey-fried Yuanzhi(Radix Polygalae)can be added to pacify the heart and calm
