奚肇庆采用消补法治疗肺结节病经验

摘　　要：奚肇庆教授将肺结节病的病机概括为:正气不足、气滞痰凝、血瘀交阻,正气不足则肺、脾、肾三脏亏虚,应治以消补法,补为扶正,消为祛邪,扶正为主,兼顾祛邪。补法常选用四君子汤、玉屏风散等,酌情选用太子参、西洋参或生晒参等以达益气养阴之效。消法常依据邪毒不同选用不同药物,热毒内蕴者用蒲公英、石见穿、白花蛇舌草、山慈菇、夏枯草、仙鹤草等清热解毒;痰气交阻者选川贝母、浙贝母、薤白、瓜蒌、桔梗、胆南星等清热化痰散结,八月札、陈皮、佛手等疏肝理气散结;兼夹瘀毒者用莪术、三棱、川芎等活血化瘀散结。在上述方药治疗的基础上,奚教授多加用中成药复方薤白胶囊以通阳宣痹,化痰散结。Professor Xi Zhao-qing has summarized the pathogenesis of pulmonary sarcoidosis as follows: insufficient healthy qi,stagnation of qi,phlegm coagulation, and obstruction of blood stasis.Insufficient healthy qi leads to deficiency of lung, spleen and kidney.It should be treated with resolving and tonifying method.Tonifying is for strengthening healthy qi,while resolving is for dispelling pathogens, focusing on strengthening healthy qi and giving consideration to dispelling pathogens.In terms of tonifying method, Four Gentlemen Decoction, Jade Wind-Barrier Powder, etc.are often used.Taizishen(Radix Pseudostellariae),Xiyangshen(Radix Panacis Quinquefolii),Shengshaishen(Radix et Rhizoma Ginseng Cruda),etc.can be properly selected for supplementing qi and nourishing yin.In terms of resolving method, different drugs are often selected according to different pathogens and toxins.For internal heat toxin accumulation, Pugongying(Herba Taraxaci),Shijianchuan(Herba Salviae Chinensis),Baihuasheshecao(Herba Hedyotis Diffusae),Shancigu(Pseudobulbus Cremastrae seu Pleiones),Xiakucao(Spica Prunellae),Xianhecao(Herba Agrimoniae),etc.can be used to clear heat and removing toxin.For those with phlegm and qi obstruction, Chuanbeimu(Bulbus Fritillariae Cirrhosae),Zhebeimu(Bulbus Fritillariae Thunbergii),Xiebai(Bulbus Allii Macrostemi),Gualou(Fructus Trichosanthis),Jiegeng(Radix Platycodonis),Dannanxing(Rhizoma Arisaematis cum Bile),etc.can be used to clear away heat, resolve phlegm and disperse stagnation.Bayuezha(Fructus Akebiae),Chenpi(Pericarpium Citri Reticulatae),Foshou(Fructus Citri Sarcodactylis),etc.can be used to soothe liver, regulate qi and disperse stagnation.For those combined with blood stasis and toxin, Ezhu(Rhizoma Curcumae),Sanleng(Rhizoma Sparganii),Chuanxiong(Rhizoma Chuanxiong),etc.are used to promote blood circulation, remove blood stasis and disperse stagnation.On the basis of the above formulas and medicinals treatment, Professor Xi often adds Compound Xiebai Capsules, a Chinese patent medicine, to unblock yang
