灸法治疗神经源性膀胱研究进展

摘　　要：神经源性膀胱病位在膀胱,与肾、三焦的功能失职有关,故治疗应调节脏腑阴阳,温通经气,扶正祛邪。灸法可助膀胱发挥气化功能,改善神经源性膀胱引起的排尿或储尿障碍。单一灸法治疗神经源性膀胱包括艾灸、隔物灸、益元灸、任脉灸、脐灸、热敏灸、温针灸等;灸法联合其他疗法包括灸法联合针刺、灸法联合中药热敷、灸法结合揿针、灸法联合中药治疗等。目前,缺少随机、多中心、大样本的灸法随机对照实验,样本量小;灸法的时间和疗程长短不一,灸感的标准有所不同,选取的穴位缺少辨证,未与疾病证型相结合;规范操作和评价指标也未形成统一标准。今后可开展大样本、多中心的随机对照研究,并依据疾病的证型科学选穴,灸法灸量应科学考量,按灸法操作规范严格执行,保证治疗效果。Neurogenic bladder disease is located in the bladder, which is related to the dysfunction of the kidney and three energizers.Therefore, the treatment should focus on regulating the yin and yang of viscera, warming and dredging meridian qi,and strengthening healthy qi and eliminating pathogenic factors.Moxibustion can help the bladder play its qi transformation function and improve the urination or urine storage disorder caused by neurogenic bladder.Single moxibustion for neurogenic bladder includes moxibustion, moxibustion for promoting original qi,moxibustion at CV,umbilical moxibustion, heat sensitive moxibustion, and warming needle moxibustion.Moxibustion combined therapy includes moxibustion combined with acupuncture, moxibustion combined with pyretic application with Chinese medicinals, moxibustion combined with thumbtack needle.At present, there is a lack of randomized, multicenter, large sample moxibustion randomized controlled trials with small sample size.The time length and course of treatment of moxibustion are various, the standard of moxibustion sensation is also different, the selected acupoints are lack of syndrome differentiation, and are not combined with disease syndrome types.Standardized operation and evaluation indicators have not formed a unified standard.In the future, it is necessary to carry out a large sample, multi center randomized controlled study, and select acupoints scientifically according to the syndrome type of the disease.The amount of moxibustion should be considered scientifically and strictly implemented based on the moxibustion operation specifications, so as to ensure the therapeutic effect.
