摘  要:目的分析显微镜下多血管炎(MPA)合并支气管扩张(简称支扩)患者的临床特点。方法回顾性分析2009年10月至2020年2月于中国医科大学附属盛京医院确诊的15例MPA合并支扩患者的临床资料,以同期住院MPA未合并支扩的45例患者为对照。结果MPA支扩组补体C4水平高于MPA无支扩组(P<0.05),免疫球蛋白M(IgM)水平、肺部间质改变比例低于MPA无支扩组(P<0.05)。logistic回归分析示低IgM水平(OR 0.192,95%CI 0.010~0.853,P<0.05)、低肺部间质改变比例(OR 0.091,95%CI 0.010~0.853,P<0.05)是MPA合并支扩的危险因素。结论支扩可能是MPA的肺部表现之一,低IgM水平和低肺部间质改变比例是MPA合并支扩的危险因素。Objective To analyze the clinical characteristics of microscopic polyangiitis(MPA)with bronchiectasis.Methods Retrospectively 15 inpatients who had been diagnosed as MPA combined with bronchiectasis from October 2009 to February 2020.At the same time,45 MPA patients without bronchiectasis were incladed as controls.Results Complement 4(C4)in MPA with bronchiectasis group was higher than that in MPA without bronchiectasis group(P<0.05).Immunoglobulin M(IgM)and the ratio of interstitial changes of the lung in those in MPA with bronchiectasis group were lower than MPA without bronchiectasis group(P<0.05).Logistic regression anaylsis showed that low serum level of IgM(OR 0.192,95%CI 0.010-0.853,P<0.05)and low ratio of interstitial changes of the lung(OR 0.091,95%CI 0.010-0.853,P<0.05)were risk factors for bronchiectasis in MPA patients with bronchiectasis.Conclusions Bronchiectasis may be one of the lung manifestations of MPA.Low serum level of IgM and low ratio of interstitial changes of the lung may be risk factors for MPA patients with brochieactasis.
