急性胰腺炎患者外周血miRNA-16和miRNA-192表达及其与病情严重程度的相关性

摘　　要：目的:探讨急性胰腺炎(AP)患者外周血miRNA-16和miRNA-192表达及其与病情程度的相关性。方法:选取广东省第二人民医院2019年1月至2021年1月诊治的AP患者91例为研究对象,按照病情程度分为轻中度组(n=57)和重度组(n=34);另选同期健康体检者为对照组(n=50)。受试者入院24h内采集外周静脉血,分离血清后采用实时荧光定量PCR法测定血清miRNA-16和miRNA-192表达程度。比较三组急性生理与慢性健康评分(APACHEⅡ)、miRNA-16和miRNA-192表达量;采用ROC曲线分析miRNA-16和miRNA-192在AP中的诊断价值;采用Pearson分析miRNA-16和miRNA-192与APACHEⅡ评分相关性。结果:轻中度组和重度组APACHEⅡ评分明显高于对照组,且重度组APACHEⅡ评分明显高于轻中度组,P均<0.05。轻中度组和重度组血清miRNA-16和miRNA-192相对表达量明显高于对照组,且重度组血清miRNA-16和miRNA-192相对表达量明显高于轻中度组,P均<0.05。ROC曲线分析显示,miRNA-16和miRNA-192诊断AP的灵敏度分别为67.27%和72.22%,特异度分别为63.89%和72.97%。miRNA-16和miRNA-192与APACHEⅡ评分均呈明显线性正相关,P均<0.05。结论:AP患者外周血miRNA-16和miRNA-192均呈高表达,且与病情严重程度呈明显线性正相关。Objective:To explore the expression of miRNA-16 and miRNA-192 in peripheral blood of patients with acute pancreatitis(AP)and its correlation with the severity of the disease. Methods:91 patients with AP in the hospital were selected as object of study from Jan 2019 to Jan 2021,and divided into mild-to-moderate group(n=57)and severe group(n=34)according to the severity of the case.s condition,and healthy physical examination were used as the control group(n=50). The peripheral venous blood was collected within 24 hours after admission,and the serum was separated.The serum miRNA-16 and miRNA-192 were tested by real-time fluorescent quantitative PCR. The acute physiology and chronic health(APACHE Ⅱ)score and the expression of miRNA-16 and miRNA-192 were compared among the three groups. ROC curve was used to analyze the diagnostic value of miRNA-16 and miRNA-192 and AP. Pearson was used to analyze the correlation of miRNA-16 and miRNA-192 and APACHE Ⅱscore. Results:The APACHE Ⅱ score in the severe group was significantly higher than that in the mild-moderate group,and the APACHE Ⅱ in the both group was significantly higher than that in the control group,all P<0.05. The relative expression levels of serum miRNA-16 and miRNA-192 in the severe group was significantly higher than that in mild-moderate group,andthe relative expression levels of serum miRNA-16 and miRNA-192 in the both group was significantly higher than that in the control group,all P<0.05. ROC curve showed that the sensitivity of miRNA-16 and miRNA-192 was 67.27% and 72.22%,while the specificity was 63.89% and 72.97%,respectively. miRNA-16 and miRNA-192 were significantly linearly positively correlated with APACHⅡscore,P<0.05. Conclusion:The peripheral blood miRNA-16 and miRNA-192 are highly expressed in patients with AP,and they are closely related to the severity of the disease.
