毕罗芬对儿童复发性鼻出血的疗效初探

摘　　要：目的:探讨毕罗芬在儿童复发性鼻出血硝酸银烧灼术后的作用,阐明毕罗芬的临床治疗效果。方法:回顾性分析93例临床病例资料,纳入具有反复鼻出血病史且鼻中隔前下部可见粘膜血管外露的门诊儿童患者。实验组行鼻中隔粘膜硝酸银烧灼术,术后使用毕罗芬规律治疗4周。对照组仅行硝酸银烧灼术。随访两组患儿4周内的再发鼻出血例数及术区愈合情况。结果:实验组共46例,其中42例(91.3%)在随访的1周内无再发鼻出血,对照组共47例,其中33例(70.2%)1周内无再发鼻出血,统计学差异具有显著性意义(P<0.05)。两组对比后发现,实验组较对照组患儿的术区愈合时间更短(9.81±2.00天vs 12.01±1.32天),统计学差异具有显著性意义(P<0.05)。结论:与单独硝酸银烧灼术相比,硝酸银烧灼术后创面应用毕罗芬持续4周能有效减少鼻出血复发频率,缩短术后康复时间,值得临床推广。Objective:To explore the effect of biprofen in children with recurrent epistaxis after silver nitrate cauterization,and to clarify the clinical therapeutic effect of biprofen. Methods:The data of 93 clinical cases were retrospectively analyzed,and outpatient pediatric patients with a history of repeated epistaxis and submucosal blood vessels were visible in the lower part of the nasal septum were included. The experimental group underwent silver nitrate cauterization of the nasal septum mucosa,and regular treatment with biprofen for 4 weeks after the operation. The control group only underwent silver nitrate cautery. The number of recurrent epistaxis and the healing of the operation area in the two groups were followed up within 4 weeks. Results:There were 46 cases in the experimental group,of which 42 cases(91.3%)had no recurrent epistaxis within one week of follow-up. There were 47 cases in the control group,of which 33 cases(70.2%)had no recurrent epistaxis within one week of follow-up,and the statistical difference was significant(P<0.05). After comparing the two groups,it was found that the experimental group had shorter healing time than the control group(9.5 days vs 13.3 days,P<0.05). Conclusion:Compared with silver nitrate cautery alone,the application of biprofen to the wound after silver nitrate cautery for 4 weeks can effectively reduce the frequency of epistaxis recurrence and shorten the postoperative recovery time,which is worthy of clinical promotion.
