通心络胶囊治疗PCI术后心绞痛效果的Meta分析

摘　　要：目的:系统评价通心络胶囊治疗经皮冠状动脉介入术(percutaneous coronary intervention,PCI)后心绞痛的疗效。方法:全面检索中国期刊全文数据库(CNKI)、期刊数据库(Wanfang Data)、维普中文科技期刊全文数据库(VIP)、PubMed、SinoMed等数据库,检索时限均为从建库至2020年8月20日。按Cochrane评价员手册5.1.0评价符合纳入标准的文献质量,采用Review Manager 5.3软件进行分析。结果:共纳入19篇文献,合计1 879例研究对象。结果显示,通心络胶囊联合常规西药(试验组)与常规西药组(对照组)比较,试验组可显著提高PCI术后心绞痛疗效[OR=3.99,95%CI(2.82,5.65),P<0.000 01],降低心绞痛复发率[OR=0.25,95%CI(0.14,0.46),P<0.000 01],减少心绞痛发作时间[MD=-1.40,95%CI(-1.55,-1.24),P<0.000 01]和频率[MD=-2.46,95%CI(-3.29,-1.64),P<0.000 01],提高心电图疗效[OR=2.22,95%CI(1.32,3.76),P=0.003]。结论:基于当前证据,在常规西药治疗的基础上,加用通心络胶囊治疗PCI术后心绞痛有更好的疗效,能有效降低心绞痛复发率,改善患者心绞痛症状。Objective:To systematically evaluate the efficacy of Tongxinluo Capsule in the treatment of angina pectoris after percutaneous coronary intervention (PCI).Method:Related clinical trials was screened in CNKI,VIP,Wanfang Data,PubMed and SinoMed from the date of establishment to August 20,2020.Cochrane manual 5.1.0 was used to evaluate the quality of the included literatures,and the literatures were analyzed by Review Manger 5.3 software.Result:A total of 19 literatures meeting the criteria were included,involving 1 879 patients.Comparing with conventional Western medicine group (the control group) and Tongxinluo Capsule combined with Western medicine (the treatment group) could improve the clinical efficacy[OR=3.99,95%CI(2.82,5.65),P<0.000 01],further reduce the recurrence rate of angina pectoris[OR=0.25,95%CI(0.14,0.46),P<0.000 01],reduce the time[MD=-1.40,95%CI(-1.55,-1.24),P<0.000 01]and frequency of angina pectoris attack[MD=-2.46,95%CI(-3.29,-1.64),P<0.000 01],and improve the effect of electrocardiogram[OR=2.22,95%CI(1.32,3.76),P=0.003].Conclusion:Current evidences show that Tongxinluo Capsule combined with conventional Western medicine in the treatment of angina pectoris after PCI has better clinical efficacy,and it can effectively reduce the recurrence rate of angina pectoris and improve the symptoms of angina pectoris.
