摘 要:目的:探究3D打印导航模板辅助微创髓芯减压联合可注射人工骨治疗早期股骨头坏死的临床效果。方法:选取2019年6月-2020年5月于本院治疗的早期股骨头坏死患者94例作为研究对象,均为单侧坏死,按照随机数字表法将其分为观察组(n=47)和对照组(n=47)。对照组给予传统髓芯减压植骨治疗,观察组给予3D打印导航模板辅助微创髓芯减压联合可注射人工骨治疗。比较两组术中相关指标、髋关节功能、疼痛情况、股骨头存活率及股骨头塌陷发生率。结果:观察组手术时间短于对照组,透视次数及术中出血量均少于对照组(P<0.05)。两组术后3、6、12个月髋关节疗效评分标准(Harris)评分较术前均提高,且观察组均高于对照组(P<0.05)。两组术后3、6、12个月视觉模拟评分法(visual analogue score,VAS)评分均较术前降低,且观察组均低于对照组(P<0.05)。观察组股骨头存活率高于对照组,股骨头塌陷发生率低于对照组(P<0.05)。结论:3D打印导航模板辅助微创髓芯减压联合可注射人工骨治疗早期股骨头坏死可改善患者术中相关指标及髋关节功能,缓解患者疼痛,提高股骨头存活率,减少不良股骨头塌陷发生,临床效果较好。Objective:To explore the clinical effect of 3D printed navigation template assisted minimally invasive core decompression combined with injectable artificial bone in the treatment of early necrosis of femoral head.Method:A total of 94 patients with early necrosis of the femoral head treated in our hospital from June 2019 to May 2020 were selected as the research subjects,all of whom had unilateral necrosis,and they were divided into observation group (n=47) and control group (n=47) according to random number table method.The control group was treated with traditional core decompression and bone grafting,while the observation group was treated with 3D printed navigation template assisted minimally invasive core decompression combined with injectable artificial bone therapy.The intraoperative indicators,hip function,pain,femoral head survival rate and the incidence of femoral head collapse were compared between the two groups.Result:The operation time of the observation group was shorter than that of the control group,the number of fluoroscopy and the amount of intraoperative blood loss were less than those of the control group (P<0.05).The Harris scores at 3,6 and 12 months after surgery in both groups were higher than those before surgery,and the observation group were higher than those of the control group (P<0.05).Visual analogue score (VAS) scores in both groups at 3,6,and 12 months after surgery were lower than those before surgery,and those in the observation group were lower than those in the control group (P<0.05).The survival rate of femoral head in observation group was higher than that in control group,and the incidence of femoral head collapse in observation group was lower than that in control group (P<0.05).Conclusion:3D printed navigation template assisted minimally invasive core decompression combined with injectable artificial bone in the treatment of early necrosis of the femoral head can improve the intraoperative related indicators and hip function of patients,relieve the pain of patients,impro
