手足口病患儿左心室局部收缩功能与血清半乳糖凝集素-3的相关性分析

摘　　要：目的:探究手足口病患儿左心室局部收缩功能与血清半乳糖凝集素-3(Gal-3)的相关性。方法:选择2017年1月-2020年1月于本院接受治疗的70例手足口病合并心功能改变患儿为研究组,另选取同期于本院接受治疗的30例单纯手足口病患儿为对照组。比较研究组和对照组血清学、心功能指标,包括Gal-3、NT-proBNP、LVEDd、LVEF。按照研究组患儿的LVEF将其分为HFREF组29例与HFPEF组41例,比较HFREF组与HFPEF组患儿的血清学、心功能指标。采用Pearson相关性分析探究Gal-3和NT-proBNP、LVEF、LVEDd水平的相关性。结果:研究组的Gal-3、NT-proBNP、LVEDd均明显高于对照组,LVEF明显低于对照组,差异均有统计学意义(P<0.05)。HFREF组患者的Gal-3、NT-proBNP、LVEDd均明显高于HFPEF组,LVEF明显低于HFPEF组,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,Gal-3水平与NT-proBNP、LVEDd均呈正相关,与LVEF呈负相关(P<0.05)。ROC曲线显示,Gal-3诊断手足口病患儿左心室局部收缩功能降低的AUC为0.7832[95%CI(0.6222,0.9443),P<0.05]。结论:手足口病患儿左心室局部收缩功能与其血清Gal-3水平呈现明显的相关性,可以考虑将该指标作为评估患儿左心室收缩功能的临床依据,用于预后预测及病情监测中。Objective:To investigate the correlation between local contractile function of left ventricle and serum galectin-3(Gal-3)in children with hand-foot-mouth disease.Method:A total of 70 cases of hand-foot-mouth disease complicated with cardiac function changes treated in our hospital from January 2017 to January 2020 were selected as the study group,and 30 cases of simple hand-foot-mouth disease treated in our hospital during the same period were selected as the control group.Serological and cardiac function indexes,including Gal-3,NT-proBNP,LVEDd and LVEF,were compared between the study group and the control group.The children in the study group were divided into HFREF group(n=29)and HFPEF group(n=41)according to LVEF.Serological and cardiac function indexes of HFREF group and HFPEF group were compared.Pearson correlation analysis was used to explore the correlation between Gal-3 and the levels of NT-proBNP,LVEF and LVEDd.Result:Gal-3,NT-proBNP and LVEDd in the study group were significantly higher than those in the control group,while LVEF was significantly lower than that in the control group,the differences were statistically significant(P<0.05).Gal-3,NT-proBNP and LVEDd in HFREF group were significantly higher than those in HFPEF group,and LVEF was significantly lower than that in HFPEF group,the differences were statistically significant(P<0.05).Pearson correlation analysis results showed that Gal-3 level was positively correlated with NT-proBNP and LVEDd,and negatively correlated with LVEF(P<0.05).ROC curve showed that,the AUC of reduced left ventricular local contractile function in children with hand-foot-mouth disease diagnosed by Gal-3 was 0.7832[95%CI(0.6222,0.9443),P<0.05].Conclusion:The left ventricular local contractile function of children with hand-foot-mouth disease is significantly correlated with the serum Gal-3 level,which can be considered as the clinical basis for evaluating the left ventricular systolic function of children,and used for prognosis prediction and disease monitoring.
