血液灌流联合血液透析在急性肾衰竭中的应用及对患者肾功能、炎症因子的影响

摘　　要：目的:分析血液灌流联合血液透析在急性肾衰竭中的应用及对患者肾功能、炎症因子的影响。方法:选取2018年12月-2020年12月本院收治的急性肾衰竭患者90例为研究对象。根据入院时间先后顺序,将患者分为研究组和对照组,每组45例。两组患者均进行常规对症治疗,在此基础上,对照组实施血液透析治疗,研究组实施血液灌流联合血液透析治疗。比较两组治疗效果,比较两组治疗前后的肾功能指标、炎症因子水平。结果:研究组总有效率为95.56%,明显高于对照组的75.56%,差异有统计学意义(P<0.05)。治疗前,两组的炎症因子、肾功能指标水平比较,差异均无统计学意义(P>0.05);治疗后,两组的IL-6、IL-1β、TNF-α、hs-CRP、SAA、BUN、Scr、24 h尿蛋白定量水平均显著低于治疗前,且研究组均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组的IL-10水平与治疗前比较,差异无统计学意义(P>0.05);治疗后,两组的IL-10水平比较,差异无统计学意义(P>0.05)。结论:血液灌流联合血液透析在急性肾衰竭中的应用效果较好,可有效改善患者的肾功能指标,抑制炎症因子水平,具有较高的临床应用价值。Objective:To analyze the application of hemoperfusion combined with hemodialysis in acute renal failure and its influence on patient’s renal function and inflammatory factors.Method:A total of 90 patients with acute renal failure admitted to our hospital from December 2018 to December 2020 were selected as the study subjects.According to the order of admission time,the patients were divided into study group and control group,45 cases in each group.Both groups received routine symptomatic treatment,on this basis,the control group was treated with hemodialysis,the study group was treated with hemoperfusion combined with hemodialysis.The therapeutic effects of two groups were compared,the renal function indexes and inflammatory factor levels of two groups before and after treatment were compared.Result:The total effective rate was 95.56% in the study group,which was significantly higher than 75.56% in the control group,the difference was statistically significant (P<0.05).Before treatment,comparison of inflammatory factors and renal function indexes between two groups,there were no significant differences (P>0.05).After treatment,IL-6,IL-1β,TNF-α,hs-CRP,SAA,BUN,Scr,24 h urinary protein quantification levels in both groups were significantly lower than those before treatment,the study group were lower than those in the control group,the differences were statistically significant (P<0.05).After treatment,the levels of IL-10 in two groups were compared with those before treatment,there were no significant differences (P>0.05).After treatment,comparison of IL-10 levels between two groups,there was no significant difference (P>0.05).Conclusion:Hemoperfusion combined with hemodialysis has a good effect in acute renal failure,it can effectively improve the renal function index and inhibit the level of inflammatory factors,it has high clinical application value.
