摘  要:目的:研究跨理论模型健康教育在脑卒中后偏瘫患者中的应用效果。方法:按照随机数字表法将2019年10月-2020年10月本院收治的116例脑卒中后偏瘫患者分为对照组和干预组,各58例。对照组进行常规健康教育,干预组进行跨理论模型健康教育。对比两组神经功能缺损情况、自我护理能力、自我管理行为。结果:两组干预前神经功能缺损情况、自我护理能力、自我管理行为比较,差异均无统计学意义(P>0.05)。干预1、3、6个月后,两组NIHSS评分、ESCA评分与干预前比较,差异均有统计学意义(P<0.05);干预3、6个月后,干预组NIHSS评分、ESCA评分与对照组比较,差异均有统计学意义(P<0.05)。干预6个月后,两组自我管理行为均高于干预前(P<0.05),干预组疾病管理、生活起居管理、情绪管理、人际管理、康复管理评分及我管理行为总分均高于对照组(P<0.05),但两组用药管理、饮食管理比较,差异均无统计学意义(P>0.05)。结论:跨理论模型健康教育能减轻脑卒中后偏瘫患者的神经功能缺损,提高患者的自我护理能力及自我管理行为,值得应用。Objective:To study the application effect of cross-theoretical model health education in patients with hemiplegia after stroke.Method:Using the random number table method,116 patients with hemiplegia after stroke in our hospital from October 2019 to October 2020 were divided into the control group and the intervention group,58 cases in each.The control group received regular health education,and the intervention group received cross-theoretical model health education.The neurological deficits,self-care ability and self-management behavior of two groups were compared.Result:There were no significant differences in neurological deficits,self-care ability and self-management behavior between two groups before intervention (P>0.05).At 1,3,and 6 months after intervention,the NIHSS score and ESCA score of two groups compared with those of before intervention,and the differences were statistically significant (P<0.05).At 3 and 6 months after intervention,the NIHSS score and ESCA score of the intervention group compared with the control group,and the differences were statistically significant (P<0.05).At 6 months after intervention,the self-management behaviors of two groups were higher than those of before intervention (P<0.05),and the disease management,daily life management,emotional management,interpersonal management,rehabilitation management scores and total self-management behaviors scores of the intervention group were higher than those of the control group (P<0.05),but there were no statistically significant differences in medication management and diet management between two groups (P>0.05).Conclusion:Cross-theoretical model health education can reduce neurological deficits of patients with hemiplegia after stroke,and improve patients’ self-care ability and self-management behavior,it is worthy of application.
