摘 要:目的:探究血清25-(OH)VD_(3)、Th1和Th2类细胞因子及免疫球蛋白在儿童肺炎中的检测价值。方法:选取2020年1月-2021年5月本院60例肺炎患儿为观察组,另选取同时期本院30名体检健康儿童为对照组。比较两组的血清25-(OH)VD_(3)、Th1类细胞因子(IFN-γ、IL-2及TNF-α)、Th2类细胞因子(IL-6及IL-10)及免疫球蛋白(IgA、IgG及IgM);比较观察组中不同严重程度者的上述指标,并采用Spearman秩相关系数分析上述指标与肺炎患儿严重程度的关系。结果:观察组血清25-(OH)VD_(3)与IgA均低于对照组,血清IgM高于对照组(P<0.05),而两组血清IgG比较,差异无统计学意义(P>0.05)。观察组中不同严重程度者血清25-(OH)VD_(3)、IgA、IgM比较,差异均有统计学意义(P<0.05)。严重程度越高者血清25-(OH)VD_(3)与IgA越低,IgM越高(P<0.05)。观察组中不同严重程度者IgG比较,差异均无统计学意义(P>0.05)。观察组的血清Th1、Th2类细胞因子均高于对照组P<0.05)。观察组中不同严重程度者血清Th1、Th2类细胞因子比较,差异均有统计学意义(P<0.05)。严重程度越高者血清Th1、Th2类细胞因子越高(P<0.05)。Spearman秩相关分析显示,血清25-(OH)VD_(3)及IgA与肺炎患儿严重程度呈负相关,血清Th1、Th2类细胞因子及IgM与肺炎患儿严重程度呈正相关(P<0.05)。结论:血清25-(OH)VD_(3)、Th1和Th2类细胞因子及免疫球蛋白在儿童肺炎中的检测价值较高,对于疾病严重程度有一定的反映意义。Objective:To explore the detection value of serum 25-(OH)VD_(3),Th1 and Th2 cytokines and immunoglobulin in children with pneumonia.Method:A total of 60 children with pneumonia in our hospital from January 2020 to May 2021 were selected as the observation group,and another 30 healthy children in our hospital during the same period were selected as the control group.Serum 25-(OH)VD_(3),Th1 cytokines (IFN-γ,IL-2 and TNF-α),Th2 cytokines (IL-6 and IL-10) and immunoglobulin (IgA,IgG and IgM) were compared between the two groups.The above indicators of children with different severity were compared in the observation group,and Spearman rank correlation coefficient was used to analyze the relationship between the above indicators and the severity of pneumonia in children.Result:Serum 25-(OH)VD_(3) and IgA in the observation group were lower than those in the control group,and serum IgM was higher than that in the control group (P<0.05),but there was no significant difference in serum IgG between the two groups (P>0.05).There were statistically significant differences in serum 25-(OH)VD_(3),IgA and IgM of children with different severity in the observation group (P<0.05).The higher the severity,the lower the serum 25-(OH)VD_(3) and IgA,the higher the serum IgM (P<0.05).There was no statistical significance in IgG of children with different severity levels in the observation group (P>0.05).Serum Th1 and Th2 cytokines in the observation group were higher than those in the control group (P<0.05).There were statistically significant differences in serum Th1 and Th2 cytokines of children with different severity levels in the observation group (P<0.05).The higher the severity,the higher the serum Th1 and Th2 cytokines (P<0.05).Spearman rank correlation analysis showed that serum 25-(OH)VD_(3) and IgA were negatively correlated with the severity of pneumonia,while serum Th1,Th2 cytokines and IgM were positively correlated with the severity of pneumonia (P<0.05).Conclusion:Serum 25-(OH)VD_(3),Th1 and Th2 cytokines and immunog
