注射用盐酸万古霉素致急性药物性肝损伤1例分析

摘　　要：目的通过分析注射用盐酸万古霉素导致急性药物性肝损伤的严重药品不良反应,为临床安全合理用药提供借鉴。方法以1例颅脑术后肝损伤患者为例,分析患者诊治经过查找患者肝损伤的原因,并采用因果关系评价标准表评估可能引起肝损伤的药物。结果该患者转氨酶水平显著升高可能与万古霉素有关,考虑为万古霉素导致的急性药物性肝损伤。结论医师和药师在使用万古霉素时除关注患者肾功能外,还应重点关注患者的肝功能,如患者出现肝功能损伤,应警惕万古霉素诱发的严重药品不良反应。Objective To analyze such severe adverse reactions as acute drug-induced liver injury caused by vancomycin hydrochloride.Methods One case of liver injury after craniocerebral surgery was investigated.The cause of liver injury was identified by analyzing the diagnosis and treatment of the patient,and the suspected drug was evaluated by the RUCAM scale.Results The significantly elevated transaminase level in this patient might have been related to vancomycin,which was considered to have caused the acute drug-induced liver injury.Conclusion When using vancomycin,physicians and pharmacists should pay attention to the liver function and renal function of patients.In case of liver injury,they should be alert to the serious adverse reactions caused by vancomycin.
