摘 要:目的探讨苦参素(又称氧化苦参碱,OMT)局部灌注治疗对兔腹主动脉粥样硬化的影响。方法雄性新西兰大白兔40只,行腹主动脉球囊损伤加高脂饲料喂养复制动脉粥样硬化(atheroschlerosis,AS)模型。模型成功的30只兔随机分为模型组、苦参素低剂量组,苦参素高剂量组,每组各10只。各组局部灌注给药时模型组给予0.9%NaCl/4mL/只,苦参素低、高剂量组分别给予苦参素100mg/4mL/只、150mg/4mL/只。给药4周后行光学相干断层呈像扫描(OCT)测量狭窄段血管的内膜厚度和面积、腹主动脉造影测量最小管腔面积狭窄百分比(MLA%)变化情况以及取腹主动脉最狭窄段行HE染色观察病理学改变。结果(1)腹主动脉狭窄段血管OCT扫描:模型组的内膜厚度和内膜面积都显著高于苦参素低、高剂量组(P<0.05);(2)腹主动脉造影:模型组的MLA%显著高于苦参素低、高剂量组(P<0.05);(3)苏木素-伊红(HE)染色:模型组较苦参素低、高剂量组的管腔明显变窄,内膜增厚明显,内皮不完整,内膜和中膜大量泡沫细胞,平滑肌细胞向内膜迁移和增殖。结论通过局部灌注苦参素的方式降低兔腹主动脉球囊损伤后血管内膜厚度和面积以及最小管腔面积狭窄百分比来改善兔腹主动脉粥样硬化。Objective To investigate the effect of local infusion treatment of oxymatrine on atherosclerosis of rabbit aorta.Methods Forty male New Zealand white rabbits were treated with abdominal aortic balloon injury and high-fat diet fed for atherosclerosis model.The 30 successful rabbits were randomly divided into a model group,a low-dose oxymatrine group,and a high-dose oxymatrine group,with 10 in each group.The model group was given 0.9%Nacl/4 mL/head during local perfusion.The low-and high-dose matrine groups were given 100 mg/4 mL/only and 150 mg/4 ml/only.Optical coherence tomography(OCT)was performed 4 weeks after the administration to measure the intimal thickness and area of the vessels in the stenosed segment,the change in the percentage of the minimum lumen area stenosis measured by abdominal aorta angiography,and the narrowest segment of the abdominal aorta The pathological changes were observed by HE staining.Results(1)OCT scan of stenoses of abdominal aorta:the intimal thickness and intimal area of the model group were significantly higher than those of low and high-dose matrine(P<0.05);(2)abdominal aorta angiography:the model group MLA%was significantly higher than that of the low-and high-dose matrine group(P<0.05);(3)Hematoxylin-eosin(HE)staining:the lumen of the model group was significantly narrower than that of the high-dose group and the endometrium The thickening is obvious,the endothelium is incomplete,there are a large number of foam cells in the intima and the media,and the smooth muscle cells migrate and proliferate to the intima.Conclusion The local infusion of oxymatrine can reduce the thickness and area of vascular endometrium and the minimum stenosis percentage of rabbits to improve the abdominal aortic atherosclerosis in rabbits.
