某省居民未满足的门诊服务需要现状及影响因素研究

摘　　要：目的了解某省居民未满足的门诊服务需要现状,并分析其影响因素,为降低未满足的门诊服务需要、促进卫生服务利用的公平性与可及性提供参考依据。方法研究利用该省第六次卫生服务调查数据,居民未满足的门诊服务需要用"率"描述,基于安德森卫生服务模型将可能的影响因素归类,利用二元logistic回归分析居民未满足的门诊服务需要的影响因素。结果调查地区居民门诊服务需要未满足率为16.9%。logistic回归分析结果显示:低保户、年龄较大、到最近医疗机构时间>10分钟、患有慢性病的居民,其门诊服务需要未满足率更高(P<0.05);受教育程度高、建立健康档案、自评健康状况较好的居民,其门诊服务需要未满足率较低(P<0.05)。结论在合理配置医疗卫生资源的基础上,关注重点人群的健康管理与健康促进工作,可以促进居民的就诊意识,以满足其卫生服务的需要。Objective It aims to explore the current situation of unmet needs for outpatient care among residents in a certain province and analyze the factors associated with it,to provide evidence for reducing the unmet care for outpatient care and improving the equity and accessibility of health service utilization.Methods The data from the Sixth National Health Services Survey in that province is applied.Unmet needs for outpatient services among residents are described as"rates".It classifies the associate factors according to the Anderson health service utilization Model and uses the logistic Regression model to analyze the factors influencing residents’unmet need for outpatient services.Results The rate of unmet needs for outpatient care was 16.9%in the surveyed area.The results of the logistic Regression model showed that,residents with lower-income,older age,chronic diseases,and needing more than 10 minutes to go to the nearest medical facility have a higher rate of unmet needs for outpatient care(P<0.05).Residents with a higher level of education,health records,and good self-rated health status have a lower rate of unmet needs for outpatient care(P<0.05).Conclusion On the basis of rationally allocating the medical and health resources,the health management and promotion of the key population can improve the residents’awareness of seeking medical treatment to meet their needs of health services.
