类风湿性关节炎疾病活动度与糖化血红蛋白相关性分析

摘　　要：目的:研究类风湿性关节炎(RA)活动度与糖化血红蛋白(HbA1c)水平的关系。方法:采用回顾性分析法,筛选2018年1月至2019年12月于我科住院治疗的170例初诊患者为研究对象,通过记录压痛关节数、肿痛关节数及视觉模拟量表计算DAS28评分,以此将患者按疾病活动度分为高、中、低3组;分别记录性别、年龄,分析3组疾病活动度与其关系;统计分析HbA1c水平与DAS28评分相关性;分析红细胞沉降率(ESR)、急性期反应蛋白(CRP)及类风湿因子(RF)水平与HbA1c的相关性。结果:HbA1c水平与DAS28、ESR、CRP呈正相关,与RF无明确相关关系。结论:RA活动度与HbA1c呈正相关,提示RA活动期易出现糖代谢异常,调控糖代谢可能对RA病情控制及预后有一定作用。Objective:To study the relationship between rheumatoid arthritis(RA)activity and glycated hemoglobin(HbA1 c levels.Methods:Using retrospective analysis,170 primary patients who were hospitalized in our department from January 2018 to December 2019 were screened as study subjects,and the DAS28 score was calculated by recording the number of pressure and painful joints,number of swollen and painful joints and visual analog scale,which was used to classify the disease activity into three groups of high,medium and low;gender and age were recorded separately,and disease activity of the three groups was analyzed in relation to their relationship;statistical analysis of the correlation between HbA1 c levels and DAS28 scores;correlation between erythrocyte sedimentation rate(ESR),acute phase reactive protein(CRP)and rheumatoid factor(RF)levels and HbA1 c were analyzed.Results:HbA1 c levels were positively correlated with DAS28,ESR and CRP,and there was no clear correlation with RF.Conclusion:RA activity was positively correlated with HbA1 c,suggesting that RA is prone to glucose metabolism abnormalities during the active phase of RA,and it is possible that regulation of glucose metabolism has a role in RA disease control and prognosis.
