摘 要:目的:建立人血清IgG抗-A/B的固相凝集检测方法,并进行评价。方法:提取人唾液中的分泌型A、B血型物质作为包被蛋白,以抗人IgG抗体作为二抗,以IgG抗-D致敏的O型红细胞作为指示红细胞,建立血清IgG抗-A/B的固相凝集检测方法,评价其特异性和重复性。随机选取40例O型血孕妇(20例检测IgG抗-A、20例检测IgG抗-B),同时采用固相凝集法和试管法检测抗体效价,配对t检验、Pearson相关分析比较2种方法检测结果。结果:包被抗原(A或B血型物质)不与无相应抗体血清反应,特异性良好。检测IgG抗-A的批内差异为2.19%~6.90%,批间差异为4.47%~8.15%;IgG抗-B的批内差异为3.03%~6.10%,批间差异为3.17%~8.80%;固相凝集法灵敏度高于试管法,其检测IgG抗-A/B的效价分别为试管法的5.86倍和9.19倍,差异有统计学意义(P<0.05)。IgG抗-A检测相关系数为0.700(P<0.05),IgG抗-B检测相关系数为0.638(P<0.05),2种方法检测结果显著相关。结论:成功建立基于固相凝集法的血清IgG抗-A/B检测方法,该法灵敏度和特异性高、重复性好,检测效率高,无毒性危害,具备良好实用价值。Objective:To establish and evaluate a Solid Phase Agglutination method for detection of titers of IgG anti-A/B in serum.Methods:A/B blood group substances in secretory human saliva was used as coated protein,anti-human IgG and O-type red blood cells which had been sensitized by IgG anti-D were used as secondary antibody and indicator erythrocyte,respectively. Solid Phase Agglutination method for detection of IgG-anti A/B was established,and specificity and repeatability were evaluated. 40 pregnant women(blood group O)were randomly selected(20 cases for IgG anti-A and 20 cases for IgG anti-B)and titers of antibodies by Solid Phase Agglutination method and Tube Test were determined. Paired t-test and Pearson Correlation Analysis were used to compare results of two methods.Results:Coated protein(A or B blood type substances)didn’t react with serum which contained no corresponding antibody,whose specificity was reliable. Intra-and inter-coefficients of variations of IgG anti-A were 2.19%~6.90% and 4.47%~8.15%,respectively. Intra-and inter-coefficients of variations of IgG anti-B were 3.03%~6.10% and 3.17%~8.80%,respectively. Sensitivity of Solid Phase Agglutination method was higher than that of Tube Test,and titer of IgG anti-A and anti-B was 5.86 times and9.19 times higher than that of Tube Test,respectively,with a significant difference(P<0.05). Correlation Coefficient of IgG anti-A and anti-B were 0.700(P<0.05)and 0.638(P<0.05),respectively.Conclusion:Solid Phase Agglutination method for detection of titers of IgG anti-A/B in serum has been established successfully,which was an ideal method with high sensitivity and specificity,reliable repeatability,high detection efficiency,non-toxic harm and good practical value.
