疏肝利胆通泄汤对慢性胆囊炎伴胆结石患者的治疗效果分析

摘　　要：目的:分析疏肝利胆通泄汤对慢性胆囊炎伴胆结石患者的治疗效果。方法:89例慢性胆囊炎伴胆结石患者随机分为观察组(45例)和对照组(44例)。对照组采用熊去氧胆酸进行治疗,观察组在对照组的基础上联合疏肝利胆通泄汤进行治疗,两组均治疗4周。比较两组治疗后疗效,比较两组治疗前和治疗后肝胆功能、血清胃动素(MOT)、胃泌素(GAS)、生长抑素(SS)水平、应激指标和不良反应发生情况。结果:观察组总有效率高于对照组(P<0.05)。治疗后,两组血清总胆红素(TBil)、总胆汁酸(TBA)、总胆固醇(TC)、GAS、SS、肾上腺素(E)、丙二醛(MDA)水平较治疗前均降低,且观察组低于对照组(P<0.05)。治疗后,两组血清MOT、超氧化物歧化酶(SOD)水平较治疗前升高,对照组低于观察组(P<0.05)。两组不良反应总发生率比较,观察组(4.44%)低于对照组(27.27%,P<0.05)。结论:疏肝利胆通泄汤治疗慢性胆囊炎伴胆结石,可有效升高患者血清MOT水平,降低GAS、SS水平,从而显著改善肠胃功能,同时改善患者肝胆功能,减轻机体应激反应,其疗效较好,同时具有较高安全性。Objective:To analyze the therapeutic effect of Shugan Lidan Tongxie Decoction in the treatment of chronic cholecystitis with gallstones.Methods:A total of 89 cases of chronic cholecystitis with gallstones were randomly divided into the control group(n=44)and the observation group(n=45).The control group was treated with Ursodeoxycholic Acid;on which basis,the observation group was also treated with Shugan Lidan Tongxie Decoction.Both groups were treated for 4 weeks.The therapeutic effect was compared after 4 weeks of the treatment between the two groups.The liver and gallbladder functions,serum levels of motilin(MOT),gastrin(GAS),somatostatin(SS)and stress indexes before and after 4 weeks of the treatment were compared in the two groups.The occurrence of adverse reactions were also compared between the two groups.Results:The total effective rate of the observation group was 93.33(42/45),which was significantly higher than 77.27%(34/44)of the control group(P<0.05).After 4 weeks of the treatment,the serum levels of total bilirubin(TBil),total bile acid(TBA),total cholesterol(TC),GAS,SS,epinephrine(E)and malondialdehyde(MDA)were lower than those before the treatment in the two groups;of which the observation group were lower than the control group(P<0.05).The serum levels of MOT and superoxide dismutase(SOD)were significantly increased after the treatment compared to those before the treatment,and the increases in the observation group were more significant(P<0.05).The total incidence of the adverse reactions in the observation group was 4.44%(2/45),which was significantly lower than 27.27%(12/44)in the control group(P<0.05).Conclusion:Shugan Lidan Tongxie Decoction can effectively increase the serum level of MOT and reduce the levels of GAS and SS,thus significantly improving gastrointestinal function and improving liver and gallbladder function,as well as alleviating stress response of the body in the treatment of chronic cholecystitis with gallstenes.The therapy has a good curative effect and a high safety.
