银蜕丸联合自血疗法治疗血热型银屑病及对患者血清肿瘤坏死因子-α、白细胞介素-8及T淋巴细胞水平的影响

摘　　要：目的:观察银蜕丸联合自血疗法治疗血热型寻常型银屑病的临床疗效,探讨其对患者血清肿瘤坏死因子-α、白细胞介素-8、T淋巴细胞免疫功能的影响。方法:160例血热型寻常型银屑病患者随机分成两组,治疗组80例给予口服自制制剂银蜕丸,每次9 g,每日三次,外用自血疗法,每周一次,取曲池、足三里穴;对照组80例给予口服消银颗粒,每次6 g,每日三次,外用自血疗法。两组患者均治疗4周为1个疗程,3个疗程后观察临床指标的变化。分别于初诊和治疗结束后,对治疗组患者进行外周血肿瘤坏死因子-α、白细胞介素-8、细胞免疫功能指标(CD4^(+)、CD8^(+))的检测,并与20例健康成人志愿者做对照。结果:两组治疗后均有一定临床疗效,治疗组结果明显优于对照组(P <0.05)。治疗后,治疗组血清肿瘤坏死因子-α、白细胞介素-8、CD8^(+)水平明显高于健康对照组,CD4^(+)低于健康对照组,差异有统计学意义(P <0.05),治疗组治疗后血清肿瘤坏死因子-α、白细胞介素-8、CD8^(+)水平降低,CD4^(+)升高,且与治疗前有显著差异(P <0.05)。结论:银蜕丸联合自血疗法治疗血热型寻常型银屑病疗效显著,且能降低银屑病患者血清肿瘤坏死因子-α、白细胞介素-8水平,改善T淋巴细胞免疫功能,提示药物作用机制,临床值得推广。Objective: To observe the clinical efficacy of Yintui Wan(银蜕丸) with autohemotherapy on blood-heat type psoriasis vulgaris. Methods: 160 patients were randomly divided into two groups. 80 patients in the treatment group were given Yintui Wan(9 g each time, three times a day), and topical autohemotherapy(once a week) at Quchi(曲池) and Zusanli(足三里) points. 80 patients in the control group were given the Xiaoyin granules(消银颗粒)(6 g each time, three times a day), and autohemotherapy. Both groups were treated for 3 courses, 4 weeks as a course. Results: Both groups had a certain clinical effect after treatment, and efficiency in the treatment group were significantly better than the control group(P<0.05). TNF-α, IL-8 and CD8^(+) levels in the treatment group were significantly higher than the control group, and CD4^(+) was lower than the control group(P<0.05). Conclusion: Yintui Wan with autohemotherapy has a significant effect on blood-heat type psoriasis vulgaris, and can reduce the serum TNF-α and IL-8 levels, improve the immune function of T lymphocytes, and is worthy of promotion.
