固冲汤治疗青春期异常子宫出血的Meta分析

摘　　要：目的:系统评价固冲汤治疗青春期异常子宫出血的临床疗效及安全性。方法:通过计算机检索中国知网、万方、维普网及PubMed数据库,纳入固冲汤治疗青春期异常子宫出血的临床随机对照试验(Randomized Controlled Trials,RCTs),文献检索时间为建库至2020年7月,由2名研究者独立对筛选文献及数据提取,采用Cochrane评价手册5.1.0对纳入文献进行质量评价,采用Revman5.3软件进行Meta分析。结果:最终纳入11篇文献,共984名受试者,Meta分析结果显示:总有效率OR=3.69,95%CI[2.40,5.65],整体效果Z=5.99,P <0.000 01;APTT OR=-3.02,95%CI[-3.06,-2.43],整体效果Z=10.16,P<0.000 01;PT OR=-1.39,95%CI[-1.68,-1.11],整体效果Z=9.64,P <0.000 01,表明固冲汤治疗青春期异常子宫出血疗效显著,和对照组相比差异具有统计学意义。结论:固冲汤治疗青春期异常子宫出血临床疗效明显,但因纳入文献样本量小、整体质量水平较低,存在一定的偏倚风险,希望今后对更大样本、高质量的RCTs试验进行更深入研究。Objective: To systematically evaluate the clinical efficacy and safety of Guchong decoction( 固 冲 汤 )on adolescent abnormal uterine bleeding. Methods: The randomized controlled trials(RCTS) of Guchong decoction in the treatment of adolescent abnormal uterine bleeding were collected by searching PubMed, CNKI, VIP and Wanfang databases. The time of literature retrieval is from the establishment of the database to July 2020. The literature was screened and extracted by two independent researchers. The quality of the included literature was evaluated by Cochrane Evaluation Manual 5.1.0, and RevMan 5.3 software was used for Meta-analysis of the included literature data. Results: A total of 984 subjects were included in 11 literatures. Meta-analysis results showed that the total effective rate OR=3.69,95%CI[2.40,5.65], and the overall effect Z=5.99, P<0.000 01. APTT OR=-3.02,95%CI[-3.06,-2.43], overall effect Z=10.16,P<0.000 01;PT OR=-1.39,95%CI[-1.68,-1.11],overall effect Z=9.64, P<0.000 01, indicating that the effect of Guchong decoction in the treatment of adolescent abnormal uterine bleeding was significant, and the difference was statistically significant compared with the control group. Conclusion: The efficacy of the Guchong decoction on adolescent abnormal uterine bleeding is exact. However, due to the small sample size and low overall quality level in the included literature, there is a risk of bias. It is hoped that a larger sample size and highquality RCTs will be further studied in the future.
