经岩骨乙状窦前入路治疗岩斜区脑膜瘤的效果

摘　　要：目的分析经岩骨乙状窦前入路治疗岩斜区脑膜瘤的效果。方法对该院神经外科2013年2月—2019年12月收治的54例岩斜区脑膜瘤患者进行观察,按数字法随机分组,各27例。观察组经岩骨乙状窦前入路治疗,对照组采用小脑幕入路治疗。比较两组患者手术切除程度、神经功能缺损程度与围术期并发症。结果两组患者治疗前神经功能缺损程度评分,差异无统计学意义(P>0.05);术后,观察组患者神经功能缺损程度评分(13.24±2.04)分低于对照组患者的(18.07±1.92)分,差异有统计学意义(t=8.959,P<0.05);围术期并发症发生率7.41%,低于对照组的29.63%,差异有统计学意义(χ^(2)=4.418,P<0.05);手术切除率更高,差异有统计学意义(Z=5.121,P<0.05)。结论经岩骨乙状窦前入路治疗岩斜区脑膜瘤疗效确切,可以有效修复患者受损神经,减轻术后并发症。Objective To analyze the effect of petrous anterior sigmoid sinus approach in the treatment of meningioma in petroslinal area.Methods 54 cases of petroclinic meningiomas admitted to the hospital from February 2013 to December 2019 were observed and randomly divided into groups with 27 cases in each group.The observation group was treated by the petrosigmoid presigmoid sinus approach,and the control group was treated by the tentorium cerebellar approach.The degree of surgical resection,the degree of neurological deficits and perioperative complications were compared between the two groups.Results There was no significant difference in the scores of neurological deficits between the two groups before treatment(P>0.05).After surgery,the scores of neurological deficits in the observation group(13.24±2.04)points were lower than those in the control group(18.07±1.92)points,and the difference was statistically significant(t=8.959,P<0.05);the perioperative complication rate 7.41%was lower than the control group's 29.63%,the difference was statistically significant(χ^(2)=4.418,P<0.05);the surgical resection rate was higher,the difference was statistically significant(Z=5.121,P<0.05).Conclusion The petrosigmoid anterior sigmoid sinus approach is effective in treating petroclinic meningioma,which can effectively repair the damaged nerves and reduce postoperative complications.
