中医综合疗法在瘀血阻滞证型膝骨性关节炎中的疗效

摘　　要：目的:探讨中医综合疗法在瘀血阻滞证型膝骨性关节炎中的临床疗效。方法:选取2017年1月-2020年1月于无锡第二中医医院就诊收治的80例瘀血阻滞证型膝骨性关节炎患者为研究对象,根据随机数表法分为对照组和观察组,每组各40例,对照组给予注射透明质酸钠及血栓通注射液等常规治疗,观察组在此基础上给予中医定向透药(丹红散)、中药熏蒸外治等中医综合治疗。于治疗4周后比较两组患者临床疗效,采用视觉模拟评分法(Visual Analogue Scale,VAS)评分评判疼痛情况,采用西安大略和麦克马斯特大学骨关节炎指数(Osteoarthritis Index of Western Ontario and McMaster University,WOMAC)评分评判关节功能情况,采用中医证候积分比较膝关节症状情况,采用36条简明健康状况评价表(36 Short Form Health Evaluation Scale,SF-36)评判生活质量。结果:观察组治疗总有效率优于对照组,治疗后两组患者VAS评分、WOMAC评分以及中医证候积分均减少,且观察组VAS评分、WOMAC评分以及中医证候积分均低于对照组(P <0.05);治疗后两组患者SF-36各维度评分均提高,且观察组SF-36各维度评分升高更显著(P <0.05)。结论:中医综合疗法在治疗瘀血阻滞证型膝骨性关节炎中疗效确切,能明显提高患者膝关节功能,并提高生活质量。Objective: To explore the clinical effect of TCM comprehensive therapy on the blood stasis obstructed type knee osteoarthritis. Methods: 80 cases from January 2017 to January 2020 in the hospital were selected as the research objects. According to the random number table method, they were divided into the control group and the observation group, 40 cases in each group. The control group was given routine treatment such as injection of sodium hyaluronate and the Xueshuantong injection(血栓通注射液). In addition to this, the observation group was given TCM comprehensive therapy, including TCM medicine directed penetration Danhong San(丹红散), TCM medicine fumigation and so on. The treatment lasted for 4 weeks. Results: The total efficiency in the observation group was better than that in the control group(P<0.05). After treatment, the VAS score, WOMAC score and TCM syndrome score in the groups were all decreased, and the above 3 indicators in the observation group were lower than the control group(P<0.05). After treatment, the SF-36 scores in both groups were increased, and in the observation group were increased more significantly(P<0.05). Conclusion: TCM comprehensive therapy on the blood stasis obstructed type knee osteoarthritis is effective, can significantly improve the knee joint function of patients, and improve the quality of life.
