2型糖尿病并发糖尿病足患者溃疡创面微生物分布及药敏结果分析

摘　　要：目的探究2型糖尿病并发糖尿病足患者溃疡创面微生物分布及药敏结果。方法甄选2020年1—12月该院收治的2型糖尿病合并糖尿病足患者108例,应用全自动细菌鉴定与药敏分析仪以及相关配套设备对患者进行糖尿病足溃疡创面微生物进行检验。结果108例2型糖尿病合并糖尿病足患者实施足部溃疡分泌物培养,分离出111株细菌,其中革兰阴性菌65株,革兰阳性菌43株,真菌3株。美罗培南、亚胺培南、阿米卡星、头孢哌酮对肺炎克雷伯菌与奇异变性杆菌有一定干预作用;美罗培南、氨苄西林、阿米卡星、环丙沙星、头孢哌酮、亚胺培南可用于对龈沟肠杆菌治疗;美罗培南与亚胺培南可对大肠埃希菌进行治疗;奇头孢呲肟、阿米卡星、头孢哌酮可对铜绿假单胞菌实施干预。金黄色葡萄糖菌对万古霉素、利奈唑胺、达福速普汀敏感性较高;表皮葡萄球菌对万古霉素、利福平、利奈唑胺、达福速普汀敏感性较高;粪肠球菌对万古霉素与利奈唑胺敏感性较高。结论予以2型糖尿病并发糖尿病足患者微生物学检测与药敏试验可减低耐药菌株产生,抑制疾病恶化,为临床治疗选药提供有效依据。Objective To investigate the microbial distribution and drug sensitivity results of ulcer wounds in patients with type 2 diabetes complicated with diabetic foot.Methods A selection of 108 patients with type 2 diabetes and diabetic foot admitted to the hospital from January to December 2020 were used to test the microbes of the diabetic foot ulcer wounds with a fully automatic bacterial identification and drug sensitivity analyzer and related supporting equipment.Results 108 patients with type 2 diabetes and diabetic foot were cultured with foot ulcer secretions.111 strains of bacteria were isolated,including 65 gram-negative bacteria,43 gram-positive bacteria,and 3 fungi.Meropenem,imipenem,amikacin,and cefoperazone have certain intervention effects on Klebsiella pneumoniae and Bacillus mirabilis;meropenem,ampicillin,amikacin,ciprofloxacin,cefoperazone,Imipenem can be used for the treatment of Enterobacter gingivalis;Meropenem and imipenem can be used for the treatment of Escherichia coli;Cephalosporin,amikacin,cefoperazone can be used for the treatment of Pseudomonas aeruginosa Implement the intervention.S.aureus is more sensitive to vancomycin,linezolid and dalfopristin;Staphylococcus epidermidis is more sensitive to vancomycin,rifampicin,linezolid and dalfopristin;Enterococcus faecalis is sensitive to vancomycin and linezolid.Conclusion Microbiological testing and drug susceptibility test for patients with type 2 diabetes complicated with diabetic foot can reduce the production of drug-resistant strains,inhibit disease progression,and provide an effective basis for clinical treatment of drugs.
