摘  要:目的探究额窦口和鼻丘气房(agger nasi cell,ANC)的形态学与慢性单侧孤立性额窦炎发生、发展的相关性。方法选取42例诊断为慢性单侧孤立性额窦炎的患者,进行额窦和ANC的CT三维重建,并测量相关参数分析患侧与健侧额窦口相关参数与ANC大小之间有无差异。结果①ANC的体积、表面积均与FI(额嘴最突出的部分与额窦后壁之间的最短距离)呈正相关(P<0.05);②患侧的FB(额嘴的厚度)、ANC的上下径较大,FI值,FI截面积和水平截面积显著小于健侧(P<0.05);③FI截面积是影响患侧额窦口水平截面积的独立影响因素(P<0.05);④RS(额嘴最突出的部分与额窦后壁转折点之间的距离)、RS截面积和水平截面积在患侧不同额窦口分级中有显著差异(P<0.05)。结论额窦口相关参数和ANC大小与慢性单侧孤立性额窦炎的发生、发展存在一定的相关性。OBJECTIVE To explore the relationship between the morphology of the frontal sinus ostium and the agger nasi cell and the occurrence and development of chronic unilateral frontal sinusitis.METHODS Forty-two patients diagnosed with chronic unilateral frontal sinusitis were selected for 3D CT reconstruction of frontal sinus and the agger nasi cell,and relevant parameters were measured.The relevant parameter and the size of agger nasi cell were compared between the normal side and the affected side.RESULTS 1.The volume and surface areaof agger nasi cellwere positively correlated with FI(the shortest distance between the most prominent part of the frontal beak and the posterior wall of the frontal sinus)(P<0.05);2.The FB(thickness of the frontal beak) value and the upper and lower diameter of the agger nasi cell were larger on the affected side(P<0.05),though FI value,FI cross-sectional area and horizontal cross-sectional area were significantly less than that of the healthy side(P<0.05);3.The FI cross-sectional area was an independent factor affecting the horizontal cross-sectional area of the frontal sinus(P<0.05);4.RS(thedistance between the most prominent part of the frontal beak and the turning point of the posterior wall of the frontal sinus) values,RS cross-sectional area and horizontal cross-sectional area were significantly different among different frontal sinus ostium grades(P<0.05).CONCLUSION The parameters of the frontal sinus ostium and the size of the agger nasi cell were correlated with the occurrence and development of chronic unilateral frontal sinusitis.
