摘　　要：目的研究新型个性化可调式口腔矫治器治疗轻中度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效及安全性。方法采用多中心自身对照方法,38例轻中度OSAHS患者佩戴口腔矫治器治疗1个月,记录佩戴前、后呼吸暂停低通气指数(AHI)、最低动脉血氧饱和度(lowest SaO_(2),LSaO_(2))、鼾声指数、Epworth嗜睡量表(Epworth sleepiness score,ESS)评分、鼾声视觉模拟量表(visual analogue scale,VAS)评分及口腔矫治器使用情况。结果37例OSAHS患者完成临床试验,有效率为67.6%(25/37),AHI、鼾声指数、ESS评分、鼾声VAS评分下降,LSaO_(2)上升,具有统计学意义(P<0.05)。患者主要不良反应为佩戴时轻微疼痛(1/37),佩戴时对言语功能轻微影响(4/37),佩戴时对吞咽功能轻微影响、流口水(3/37)。结论新型个性化可调式口腔矫治器治疗轻中度OSAHS具有较好的疗效及安全性,不良反应少,依从性高。OBJECTIVE To evaluate the efficacy and safety of a new customized adjustable oral appliance in the treatment of mild and moderate obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS This is a multicenter,self-control trial.A total of 38 mild and moderate OSAHS patients were treated with customised adjustable oral appliance for 1 month.Apnea hypopnea index(AHI),lowest arterial oxygen saturation(LSaO_(2)),snoring index,Epworth sleepiness scale(ESS)score,snoring visual analogue scale(VAS)score and oral appliance use were recorded before and after oral appliance treatent.RESULTS Thirty seven patients with OSAHS completed the clinical trial.The effective rate was 67.6%(25/37).The AHI,snore index,sleepiness score and snore score decreased,while LSaO_(2) increased,all of which were statistically significant(P<0.05).The main side effects were slight pain(1/37),slight influence on speech function(4/37)and on swallowing function,and drooling(3/37)when using oral appliance.CONCLUSION The new customized adjustable oral appliance had better efficacy and safety in the treatment of mild and moderate OSAHS with less side effects and higher compliance.
