重症心脏瓣膜置换术的麻醉与心肌保护分析

摘　　要：目的研究将小剂量麻醉联合磷酸肌酸钠应用于重症心脏瓣膜置换手术患者中的作用与效果。方法便利选择2019年12月—2020年12月期间该院接收的重症心脏瓣膜置换手术患者68例,依据随机化分组将其分为对照组(共34例)、观察组(共34例),对照组先予常规麻醉,而观察组予小剂量麻醉联合磷酸肌酸钠,比较两组所得的结果。结果麻醉诱导时、切皮时,观察组的平均脉动压、心率依次是(15.37±0.44)kPa、(13.38±0.24)kPa、(102.67±1.08)次/min、(89.37±0.74)次/min,对照组平均脉动压、心率依次是(17.62±0.08)kPa、(15.69±0.21)kPa、(113.60±1.29)次/min、(96.62±0.33)次/min,,差异有统计学意义(t=29.336、42.237、37.882、52.175,P<0.05);观察组心脏自动复跳率73.53%(25/34),较之于对照组心脏自动复跳率32.35%(11/34)高,差异有统计学意义(χ^(2)=11.569,P<0.05)。结论重症心脏瓣膜置换手术麻醉工作中应用小剂量麻醉联合磷酸肌酸钠能够改善患者平均脉动压、心率,并提升其心脏自动复跳率,能够获得更为理想的效果,促进其疾病康复。Objective To study the effect and effect of applying low-dose anesthesia combined with creatine phosphate sodium in patients undergoing severe heart valve replacement surgery.Methods Convenient selected 68 cases of severe heart valve replacement surgery received in the hospital from December 2019 to December 2020.According to randomization,they were divided into the control group(34 cases in total)and the observation group(34 cases in total),the control group was given conventional anesthesia first,while the observation group was given low-dose anesthesia combined with creatine phosphate sodium to compare the results of the two groups.Results During induction of anesthesia and skin incision,the average pulse pressure and heart rate of the observation group were(15.37±0.44)kPa,(13.38±0.24)kPa,(102.67±1.08)beats/min,(89.37±0.74)beats/min,the average pulse pressure and heart rate of the control group were(17.62±0.08)kPa,(15.69±0.21)kPa,(113.60±1.29)beats/min,and(96.62±0.33)beats/min,the difference was statistically significant(t=29.336,42.237,37.882,52.175,P<0.05);Observation group of the heart rate was 73.53%(25/34),which was higher than the control group's 32.35%(11/34),the difference was statistically significant(χ^(2)=11.569,P<0.05).Conclusion The application of low-dose anesthesia combined with creatine phosphate sodium in the anesthesia work of severe heart valve replacement surgery can improve the average pulse pressure and heart rate of the patient,and increase the automatic recovery rate of the heart,which can obtain more ideal results and promote the recovery of the disease.
