摘 要:目的比较地佐辛复合不同剂量比丙泊酚与依托咪酯在无痛肠镜术中临床效果和安全性。方法方便选取该院2019年5月—2021年2月收治的126例无痛肠镜检查患者,随机分3组,每组42例。A组静脉推注地佐辛2.5 mg及阿托品0.25 mg+复合剂量比为15∶1丙泊酚与依托咪酯混合物;B组地佐辛2.5 mg及阿托品0.25 mg+复合剂量比为10∶1丙泊酚与依托咪酯混合物;C组地佐辛2.5 mg及阿托品0.25 mg+丙泊酚。分别记录术后结果。结果3种配伍方法均可使手术完成。对收缩压的影响B组小于A、C组,差异有统计学意义(P<0.05);对舒张压的影响A组在T2时影响较大,但是在T3时恢复较快,差异有统计学意义(P<0.05);对心率影响A组最小,B组最明显,差异有统计学意义(P<0.05);B组对呼吸频率影响最轻微为7.14%,A组居中为14.29%,C组最大为33.33%,差异有统计学意义(χ^(2)=10.319,P=0.006);A组对血氧饱和度影响最小,C组影响最大,差异有统计学意义(P<0.05);C组的不良反应发生率42.86%明显高于A组14.29%和B组14.29%,差异有统计学意义(P<0.05)。结论A组在舒张压、心率、血氧饱和度等方面占有优势,可以减少患者血压波动和心肌耗氧,保证心脏血供,维持正常呼吸和机体供氧。Objective To compare the clinical effect and safety of dizocine combined with different dose ratios of propofol and etomidate in painless colonoscopy.Methods A total of 126 patients undergoing painless colonoscopy in the hospital from May 2019 to February 2021 were conveniently selected and randomly divided into three groups with 42 persons in each group.In group A,intravenous bolus of dezocine 2.5 mg and atropine 0.25 mg+composite dose ratio of 15∶1 propofol and etomidate;group B dezocine 2.5 mg and atropine 0.25 mg+composite dose ratio of 10∶1 propofol mixture of phenol and etomidate;group C dezocine 2.5 mg and atropine 0.25 mg+propofol.Record the postoperative results separately.Results Three compatible methods can complete the operation.The impact on systolic blood pressure in group B was smaller than that in groups A and C,the difference was statistically significant(P<0.05);the impact on diastolic blood pressure in group A was greater at T2,but the recovery was faster at T3,the difference was statistically significant(P<0.05);the impact on heart rate was the smallest in group A and the most obvious in group B,the difference was statistically significant(P<0.05);the minimal effect of respiratory frequency was 7.14%,the middle of group A was 14.29%,and the maximum of group C was 33.33%,the difference was statistically significant(χ^(2)=10.319,P=0.006).Group A had the least impact on blood oxygen saturation,and group C had the greatest impact,and the difference was statistically significant(P<0.05);the adverse reaction rate of group C was 42.86%significantly higher than that of group A and 14.29%and group B 14.29%,the difference was statistically significant(P<0.05).Conclusion Group A has advantages in diastolic blood pressure,heart rate,blood oxygen saturation,etc.It can reduce blood pressure fluctuations and myocardial oxygen consumption,ensure blood supply to the heart,and maintain normal breathing and body oxygen supply.
