肺超声评分在新生儿呼吸窘迫综合征诊断及病情严重程度评估中的应用价值

摘　　要：目的探讨双肺12区超声评分在新生儿呼吸窘迫综合征(NRDS)诊断及病情严重程度评估中的应用价值。方法98例NRDS患儿(观察组)和50例非肺病新生儿(对照组)行肺超声检查,做双肺12区超声评分。对比检查结果,分析双肺12区超声评分对NRDS病情的预测效能。结果观察组的肺实质、胸膜线异常、A线消失、弥漫性肺水肿、支气管充气征检出率高于对照组,双肺12区超声评分低于对照组(P<0.05)。不同X线分级NRDS患儿的双肺12区超声评分比较有轻度>中度>重度。NRDS患儿的X线分级与双肺12区超声评分呈负相关(r=-0.661,P<0.05)。双肺12区超声评分预测轻中度NRDS的灵敏度、特异度分别为87.54%和95.36%,预测中重度NRDS的灵敏度、特异度分别为90.66%和92.76%。结论双肺12区超声评分能较早发现NRDS,准确预测NRDS病情,为NRDS的临床诊治提供指导。Objective To investigate the application value of dual lung 12 zone ultrasound score in the diagnosis and severity evaluation of neonatal respiratory distress syndrome(NRDS).Methods 98 cases of NRDS(observation group)and 50 cases of non pulmonary disease newborns(control group)were performed pulmonary ultrasound,and double lung 12 area ultrasound score was performed.The diagnostic results were compared and the predictive efficacy of dual lung 12 zone ultrasound score for NRDS was analyzed.Results The detection rate of lung parenchyma,abnormal pleural line,disappearance of a line,diffuse pulmonary edema and bronchial inflation sign in the observation group were higher than those in the control group,and the ultrasonic score of double lung 12 areas was lower than that of the control group(P<0.05).There were mild>moderate>severe in 12 area ultrasound scores of NRDS children with different X-ray grades.There was a negative correlation between the X-ray grading of NRDS and the ultrasound score of both lungs(r=-0.661,P<0.05).The sensitivity and specificity of dual lung 12 zone ultrasound score in predicting mild to moderate NRDS were 87.54%and 95.36%,respectively,and the sensitivity and specificity of predicting moderate and severe NRDS were 90.66%and 92.76%,respectively.Conclusions Dual lung 12 zone ultrasound score can detect NRDS early,accurately predict the condition of NRDS,and provide guidance for the clinical diagnosis and treatment of NRDS.
