肺部超声评分联合血CRP、LDH水平对肺炎支原体肺炎患儿病变程度评估的临床意义

摘　　要：目的探讨肺部超声(LUS)评分联合血C反应蛋白(CRP)、乳酸脱氢酶(LDH)水平对肺炎支原体肺炎(MMP)病变程度的评估价值。方法选取82例MMP患儿作为研究对象,根据病变程度分为难治性(28例)与普通性(54例),入院3 d内行肺部超声检查并评分,统计对比两组LUS评分、CRP、LDH水平,分析难治性MMP病变程度影响因素,并绘制ROC曲线分析LUS评分、CRP、LDH水平对MMP病变程度的评估价值。结果MMP患儿左右侧LUS评分相比,无明显差异(P>0.05),后侧LUS评分高于前侧,下部LUS评分高于上部,差异有统计学意义(P<0.05);Logistic回归方程进行分析,发现LUS评分、CRP、LDH水平均为难治性MMP患儿病变程度影响因素(P<0.05);ROC曲线显示,三者联合评估AUC值最大,为0.907,最佳灵敏度、特异度分别为82.14%、90.74%。结论难治性MMP患儿LUS评分及CRP、LDH水平较高,临床可监测上述指标,评估MMP病变程度,为临床治疗方案制定提供依据。Objective To investigate the evaluation value of lung ultrasound(LUS)score combined with blood C-reactive protein(CRP)and lactate dehydrogenase(LDH)levels in evaluating the severity of Mycoplasma pneumoniae pneumonia(MMP).Methods A total of 82 children with MMP in our hospital were selected as the research objects.According to the degree of disease,they were divided into refractory(28 Cases)and general(54 Cases).Lung ultrasound examination and scoring were performed within three days of admission,,LUS score,CRP,and LDH levels of the two groups were compared statistically,the influencing factors of refractory MMP lesions were analyzed,and the ROC curve was drawn to analyze the evaluation value of LUS score,CRP,LDH levels on the severity of MMP lesions.Results There was no significant difference between the left and right LUS scores of children with MMP(P>0.05).The posterior LUS score was higher than the anterior side,and the lower LUS score was higher than the upper part,the difference was statistically significant(P<0.05).Logistic regression analysis found that all the LUS score,CRP,and LDH were the influencing factors of the degree of disease in children with refractory MMP(P<0.05);ROC curve showed that the AUC value of combined assessment was the largest as 0.907,and the best sensitivity and specificity were 82.14%and 90.74%respectively.Conclusions The LUS score,CRP and LDH levels are higher in children with refractory MMP.These indicators could be monitored clinically to assess the degree of MMP lesions and provide evidence for the clinical treatment plans.
