超声斑点追踪技术联合心肌肌钙蛋白T与有效甲状腺素比值在甲亢性心脏病中的联合诊断价值

摘　　要：目的探讨超声斑点追踪技术联合心肌肌钙蛋白T(cTnT)与有效甲状腺素比值(ETR)在甲亢性心脏病中的联合诊断价值。方法回顾性分析确诊为甲亢患者82例。将其中单纯甲亢患者42例纳入对照组;将甲亢心患者40例纳入观察组。结果观察组左室基底段环向应变(Cir-B)、左室中间段环向应变(Cir-M)、左室心尖段环向应变(Cir-A)、左室整体环向应变、左室前壁-下壁纵向应变(L-AP2)、左室侧壁-后间隔纵向应变(L-AP4)、左室前间隔-后壁纵向应变(L-AP3)、左室整体纵向应变均显著小于对照组(P<0.05);观察组cTnT、ETR水平显著高于对照组(P<0.05);ETR的曲线下面积(AUC)为0.714,灵敏度与特异度分别为75.21%、65.38%;超声斑点追踪技术的AUC为0.745,灵敏度与特异度分别为72.19%、89.32%;cTnT的AUC为0.823,灵敏度与特异度分别为90.24%、81.19%;联合诊断的AUC为0.872,灵敏度与特异度分别91.28%、90.77%。结论甲亢心患者血清cTnT、ETR水平异常升高,左心室心肌收缩功能受损,联合检测超声斑点追踪技术、cTnT、ETR对早期甲亢心的诊断效能高于单项检测。Objective To investigate the diagnostic value of speckle tracking echocardiography combined with cardiac troponin T(cTnT)and effective thyroxine ratio(ETR)in hyperthyroidism heart disease.Methods From 82 patients with hyperthyroidism in our hospital were retrospectively studied.Forty-two patients with hyperthyroidism were included in the control group,and 40 patients with hyperthyroidism and heart disease were included in the observation group.By observing the difference of speckle tracking echocardiography,cTnT and ETR between groups,and the diagnostic value of single and combined in hyperthyroidism heart disease.Results Cir-B,Cir-M,Cir-A,overall left ventricular circumferential strain,L-AP2,L-AP4,L-AP3,and overall left ventricular longitudinal strain of the observation group were significantly lower than those of the control group(P<0.05).The cTnT,ETR level in the observation group was significantly higher than that in the control group(P<0.05).The area under the curve(AUC)of ETR was 0.714,and the sensitivity and specificity was 75.21%and 65.38%,respectively.The AUC of speckle tracking echocardiography is 0.745,and the sensitivity and specificity was 72.19%and 89.32%,respectively.The AUC of cTnT was 0.823,and the sensitivity and specificity was 90.24%and 81.19%,respectively.The AUC of combined diagnosis was 0.872,and the sensitivity and specificity were 91.28%and 90.77%,respectively.Conclusions In patients with hyperthyroidism heart disease,serum cTnT and ETR levels are abnormally elevated,and left ventricular myocardial systolic function is impaired.The combined detection of speckle tracking echocardiography,cTnT and ETR is more effective in diagnosing early hyperthyroidism than single detection.
