摘　　要：目的探讨胃肠道间质瘤腹腔转移的超声特点及诊断价值。方法回顾性分析56例经病理证实为间质瘤合并腹腔转移的超声表现。结果56例腹腔转移灶累及部位:肝脏47例,网膜24例,腹膜16例,腹腔淋巴结4例,胰腺1例,脾脏1例,肾脏1例。图像多表现为边界清楚(71.4%),形态规则(94.6%),低回声为主(57.1%),伴有囊变(64.3%),血流不丰富(67.9%)。转移灶与原发灶的数目、大小、形态及有无囊变的差异具有统计学意义;肝内转移灶与腹腔转移灶大小的差异具有统计学意义。结论超声对间质瘤腹腔转移的检出及诊断具有重要意义,了解间质瘤腹腔转移特点,有助于间质瘤转移的诊断。Objective To investigate the characteristics and diagnostic value of ultrasound in gastrointestinal stromal tumors with abdominal cavity metastasis.Methods The ultrasonographic characteristics of 56 patients with metastatic gastrointestinal stromal tumors confirmed by biopsy pathology were reviewed.Results The metastasis sites in total 56 patients involved that 47 cases were liver metastasis,24 cases were omentum metastasis,16 cases were peritoneal metastasis,4 cases were abdominal lymph node metastasis,1 case was pancreas metastasis,1 case was spleen metastasis,and 1 case was kidney.The ultrasound characterized by clear boundaries(71.4%),regular morphology(94.6%),hypoechoic(57.1%),cystic degeneration(64.3%),and the inabundant blood flow(67.9%).There were statistically significant difference in the number,size,shape,and cystic degeneration between metastatic and primary lesions.The difference in the size of intrahepatic and abdominal metastases was statistically significant.Conclusions Ultrasonography played an important role in the detection and diagnosis of gastrointestinal stromal tumors with abdominal cavity metastasis.For patients with a history of gastrointestinal stromal tumors,it should be alert to the occurrence of stromal tumor metastases when ultrasound examination found new lesions in abdominal cavity.
