超声引导下置管引流治疗艾滋病合并巨大肝脓肿的应用价值

摘　　要：目的探讨超声引导下置管引流治疗艾滋病合并巨大肝脓肿的临床应用价值。方法回顾性分析超声引导下置管引流治疗20例艾滋病合并巨大肝脓肿患者的临床资料。结果(1)脓肿直径10.1~21.6 cm,平均(12.4±2.8)cm。(2)置管1周后炎症指标明显下降,2周后肝功能明显好转,脓肿缩小。(3)置管引流后治愈者16例(80%),引流后再次行外科手术治疗者4例(20%)。(4)置管引流时间5~77 d,中位数为19 d,住院时间12~90 d,中位数为29 d。(5)20例患者术后均未发生并发症及未发生医护职业暴露。结论超声引导下置管引流治疗艾滋病合并巨大肝脓肿安全、有效、损伤小、患者易耐受。Objective To explore the clinical value of ultrasound-guided drainage in the treatment of AIDS complicated with huge hepatic abscess.Methods The clinical data of 20 AIDS patients with huge hepatic abscess were analyzed retrospectively.Results(1)The diameter of hepatic abscess ranged from 10.1 cm to 21.6 cm,with an average of(12.4±2.8)cm.(2)After one week of catheterization,inflammatory indexes decreased significantly,liver function improved and abscess shrinked significantly after two weeks.(3)16 cases(80%)were cured after drainage,and 4 cases(20%)were treated by surgery after drainage.(4)The duration of drainage ranged from 5 to 77 days,with a median of 19 days,and length of hospitalization ranged from 12 to 90 days,with a median of 29 days.(5)There were no complication and occupational exposure occurred in 20 patients.Conclusions It is safe,effective and tolerable to treat patients of AIDS complicated with huge hepatic abscess by ultrasound-guided drainage.
