摘　　要：目的探讨经直肠腔内超声(TRUS)对直肠子宫内膜异位症(RE)的诊断价值。方法收集经手术确诊,并且术前行TRUS的13例RE的临床与超声资料,总结声像图特征,分析术前超声诊断与病灶浸润深度的准确度。结果13例RE,TRUS均表现为局部直肠壁不均质低回声改变,84.6%(11/13)病灶形态不规则,76.9%(10/13)病灶边界不清,Adler血流分级为0~Ⅰ级者占84.6%(11/13),Ⅱ~Ⅲ级者占15.4%(2/13)。术前TRUS诊断准确率为69.2%(9/13),评估病灶累及肠壁深度符合率为61.5%(8/13)。结论TRUS对RE的诊断具有一定价值,检查时应注意结合临床,避免误诊为直肠癌。Objective To investigate the diagnostic value of transrectal ultrasonography(TRUS)for rectal endometriosis(RE).Methods The clinical and ultrasound data of 13 patients with RE who were confirmed by pathology in the Fujian Medical University Affiliated Union Hospital and underwent TRUS before surgery were collected to summarize the sonographic features and analyze the accuracy of preoperative ultrasonic diagnosis and lesion infiltration depth.Results All 13 cases of RE and TRUS showed heterogeneous hypoechoic changes in the local rectal wall,84.6%(11/13)of the lesions were irregular in shape,76.9%(10/13)of the lesions were unclear boundary,84.6%(11/13)with Adler blood flow grading from 0-I and 15.4%(2/13)ⅡtoⅢ.The preoperative diagnostic accuracy rate of TRUS was 69.2%(9/13),and the coincidence rate of lesion involvement depth of intestinal wall was 61.5%(8/13).Conclusions TRUS has s certain value in the diagnonsis of RE,and attension should be paid to combining clinical practice to avoid misdiagnosis of rectal cancer.
