摘  要:目的探讨经皮椎间孔镜技术(PELD)治疗老年极外侧型腰椎间盘突出症(FLDH)的临床疗效和安全性。方法选择南通大学附属海安医院骨科2016年7月~2017年12月收治的老年FLDH患者72例,分为观察组和对照组,分别采用PELD和常规椎板开窗髓核摘除术,观察两组手术、椎间盘突出症及术后恢复相关指标。结果两组的手术时间、术中出血量、术后卧床时间和住院时间指标比较,差异均有统计学意义(P<0.05);视觉模拟评分(VAS)、Lehmann评分和OsWestry功能障碍指数等指标比较,差异均有统计学意义(P<0.05);随访6个月,两组的镇痛剂使用率、并发症率、复发率和恢复优良率等指标比较,差异均有统计学意义(P<0.05)。结论采用PELD治疗老年FLDH,创伤轻、恢复快、并发症少、肢体功能恢复佳。Objective To evaluate the efficacy and safety of percutaneous endoscopic lumbar discectomy(PELD)in the treatment of older patients with far-lateral lumbar disc herniation(FLDH).Methods 72 older FLDH patients admitted to the Department of Orthopedics of Hai’an Hospital Affiliated to Nantong University were enrolled from July 2016 to December 2017.The patients were divided into the observation group(treated with PELD)and the control group(treated with fenestration discectomy).The surgery-related data,follow-up information and the lumbar vertebral function were documented.Results There were significant differences in operation time,intraoperative blood loss,postoperative bed rest time and hospitalization time between the two groups(P<0.05).There were significant differences between the two groups in visual analogue scale(VAS)score,Lehmann score and OsWestry disability index(ODI)(P<0.05).There were significant differences in the rate of analgesics use,incidence of complication and recurrence,rate of excellent and good recovery(P<0.05).Conclusion PELD was effective in the treatment of older FLDH patients with minimal trauma,quicker recovery,fewer complications and better recovery of limb function.
