单一髂腹股沟入路治疗复杂髋臼骨折的方法及疗效分析

摘　　要：目的分析单一髂腹股沟入路治疗复杂髋臼骨折的方法及疗效。方法该次研究采用回顾性分析方式共对该院创伤外科2016年2月-2019年5月期间入院的56例复杂髋臼骨折患者进行观察,按照手术入路差异分组,对照组应用前后入路翻修治疗,观察组应用单一髂腹股沟入路治疗,比较两组疗效。结果观察组手术时间(3.1±0.3)h,手术失血量(852.6±72.7)mL,术中输血量(705.2±82.6)mL,手术切口(23.8±3.2)cm,数据优于对照组,差异有统计学意义(t=7.432、8.416、9.887、10.732,P<0.05);观察组关节功能恢复优良率是96.43%(27例),对照组是71.43%(20例),差异有统计学意义(χ^(2)=4.766,P<0.05);观察组术后CRP(4.23±1.42)mmol/L,TNF-α(7.28±2.76)ng/L,IL-6(70.18±5.83)ng/L,数据低于对照组,差异有统计学意义(t=3.902、4.402、5.417,P<0.05)。结论单一髂腹股沟入路治疗复杂髋臼骨折手术损伤相对较小,术中出血量少,对关节功能恢复有利,且术后炎性反应较轻。Objective To analyze the method and efficacy of a single iliac-groin approach in the treatment of complex acetabular fractures.Methods The study used a retrospective analysis method to observe 56 patients with complex acetabular fractures admitted to the Department of Traumatology in the hospital from February 2016 to May 2019.According to surgical approach differences,the control group of anterior and posterior approach was used for revision therapy,and the observation group was treated with a single iliac-groin approach to compare the efficacy of the two groups.Results The operation time of the observation group was(3.1±0.3)h,the surgical blood loss was(852.6±72.7)mL,the intraoperative blood transfusion was(705.2±82.6)mL,and the surgical incision was(23.8±3.2)cm.The data were better than those of the control group,the difference was statistically significant(t=7.432,8.416,9.887,10.732,P<0.05);The excellent rate of joint function recovery in the observation group was 96.43%(27 cases),and the control group was 71.43%(20 cases),the data difference was statistically significant(χ^(2)=4.766,P<0.05);The postoperative CRP of the observation group was(4.23±1.42)mmol/L,TNF-αwas(7.28±2.76)ng/L,and IL-6 was(70.18±5.83)ng/L.The data was lower than that of the control group,the difference was statistically significant(t=3.902,4.402,5.417,P<0.05).Conclusion The single iliac-groin approach for the treatment of complex acetabular fractures has relatively small surgical damage and less intraoperative blood loss.It is beneficial to the recovery of joint function,and the postoperative inflammatory reaction is lighter.
