摘　　要：目的探讨克氏针临时固定+钢板固定治疗粉碎性锁骨骨折的疗效。方法将50例粉碎性锁骨骨折患者按照手术方式不同分为对照组(采用钢板固定治疗,25例)和观察组(采用克氏针临时固定+钢板固定治疗,25例)。比较两组手术时间、术中出血量、术中透视次数、骨折愈合时间。采用Dawson评分标准评价肩关节功能。结果患者均获得随访,时间12~18个月。手术时间、术中出血量观察组明显短(少)于对照组(P<0.05);术中透视次数、骨折愈合时间两组比较差异均无统计学意义(P>0.05)。术后1周及3、10个月Dawson评分两组比较差异均无统计学意义(P>0.05)。结论与钢板固定相比,克氏针临时固定+钢板固定治疗粉碎性锁骨骨折可以取得同样满意的疗效,具有手术时间短、术中出血量少的优势。Objective To explore the curative effect of Kirschner wire temporary fixation plus plate fixation for treatment of comminuted clavicle fracture.Methods According to different surgical methods,50 patients with comminuted clavicle fractures were divided into control group and observation group,with 25 cases in each group.The control group was treated with plate fixation,and the observation group was treated with Kirschner wire temporary fixation plus plate fixation.The operative time,intraoperative blood loss,intraoperative fluoroscopy times,fracture healing time were compared between the two groups.The shoulder function was evaluated with Dawson score criterion.Results All patients were followed up for 12~18 months.Compared with the control group,the operation time was shorter and the intraoperative blood loss was less in the observation group(P<0.05);there were no significant differences in the intraoperative fluoroscopy times and fracture healing time between the two groups(P>0.05).The Dawson scores at 1 week,3 months and 10 months after surgery had no significant differences between the two groups(P>0.05).Conclusions Compared with conventional plate fixation,Kirschner wire temporary fixation plus plate fixation can also achieve satisfactory effect for treatment of comminuted clavicle fracture,which have the advantages of shorter operation time and less intraoperative blood loss.
