两种内固定治疗老年股骨转子间骨折的疗效比较

摘　　要：目的比较股骨近端防旋髓内钉(PFNA)和InterTan髓内钉治疗老年股骨转子间骨折的疗效。方法将40例老年股骨转子间骨折患者根据内固定方式不同分为PFNA组(PFNA治疗,20例)和InterTan组(InterTan髓内钉治疗,20例)。比较两组术中出血量、手术时间、住院时间、骨折愈合时间以及髋关节功能Harris评分。结果术中出血量、手术时间、住院时间、术中透视时间PFNA组少(短)于InterTan组(P<0.05),两组术后均无并发症发生。患者均获得12个月随访。骨折愈合时间PFNA组短于InterTan组(P<0.05)。Harris评分:两组术后12个月与术后3个月比较差异均无统计学意义(P>0.05);术后3、12个月两组比较差异均无统计学意义(P>0.05)。结论采用PFNA和InterTan髓内钉治疗老年股骨转子间骨折患者均能获得良好疗效,但PFNA内固定手术时间更短、术中出血量更少、骨折愈合时间更短。Objective To compare the efficacy of proximal femoral nail anti-rotation(PFNA)and InterTan intramedullary nail in the treatment of intertrochanteric fractures in the elderly.Methods The 40 elderly patients with intertrochanteric fractures were divided into observation group(20 cases were treated with PFNA)and InterTan group(20 cases were treated with InterTan intramedullary nail),according to the different internal fixation methods.The intraoperative bleeding volume,operation time,hospital stay time,fracture healing time and Harris score of hip function were compared between the two groups.Results The intraoperative bleeding volume,operation time,hospitalization time and intraoperative fluoroscopy time in PFNA group were less(shorter)than those in InterTan group(P<0.05),there were no postoperative complications in both groups.All patients were followed up for 12 months.The fracture healing time in the PFNA group was shorter than that in the InterTan group(P<0.05).Harris score:there was no differences in the two groups at 3,12 months postoperation(P>0.05);and there were no statistical differences between the two groups at 12 months and 3 months postoperatively(P>0.05).Conclusions Both of PFNA and InterTan intramedullary nail can obtain good efficacy in the treatment of femoral intertrochanteric fracture in the elderly,but PFNA fixation has shorter operation time,less blood loss,shorter fracture healing time.
