两种内固定方式治疗股骨转子间骨折的疗效比较

摘　　要：目的比较两种内固定方式治疗股骨转子间骨折的疗效。方法将46例股骨转子间外侧壁破裂型骨折患者按照内固定方式不同分为对照组(采用股骨近端防旋髓内钉治疗,23例)和观察组(采用InterTan髓内钉治疗,23例)。记录两组手术情况、术后并发症发生情况、术后功能恢复情况。结果患者均获得12个月随访。术中闭合复位时间、手术时间、开始负重时间、术后并发症发生率观察组明显短(低)于对照组(P<0.05);术中出血量、骨折愈合时间两组比较差异均无统计学意义(P>0.05)。末次随访时Harris评分及优良率观察组均明显高于对照组(P<0.05)。结论与采用股骨近端防旋髓内钉治疗相比,采用InterTan髓内钉治疗股骨转子间外侧壁破裂型骨折临床效果更好,术后并发症发生率较低,整体治疗安全性较好,同时可帮助股骨头颈恢复正常的解剖学结构,促进骨折愈合,更利于患者提高生活质量。Objective To compare the effect of two kinds of internal fixation methods in the treatment of femoral intertrochanteric fracture.Methods The 46 patients with femoral intertrochanteric lateral wall fractures were divided into control group(23 cases were treated with proximal femoral nail anti-rotation)and observation group(23 cases were treated with InterTan intramedullary nail).The operation condition,postoperative complications situation and postoperative functional recovery of the two groups were recorded.Results All patients were followed up for 12 months.The time of closed reduction during operation,operation time,start time of weight-bearing and the incidence rate of complications in the observation group were significantly shorter(lower)than those in the control group(P<0.05);there were no significant differences in intraoperative blood loss and fracture healing time between the two groups(P>0.05).At the last follow-up,Harris score and its excellent-good rate in the observation group were significantly higher than those in the control group(P<0.05).Conclusions Compared with proximal femoral nail anti-rotation,InterTan intramedullary nail has better clinical effect in the treatment of femoral intertrochanteric lateral wall fracture,with lower incidence rate of postoperative complications,better overall treatment safety,and which can help the femoral head and neck to restore the normal anatomic structure,promote fracture healing,and improve the quality of life of patients.
