摘 要:目的探究人肿瘤坏死因子α诱导蛋白3(TNFAIP3)相互作用蛋白1(TNIP1)基因多态性与狼疮性肾炎(LN)的关系。方法选取2017年1月-2019年3月安庆市第一人民医院肾内科经肾活检确诊为LN的患者105例,并募集同期体检正常者56名作为对照组,采集受试者肘部静脉血5 ml,选取位于TNIP1基因区域的单核苷酸多态性(SNP)位点(rs7708392、rs10036748及rs13168551),采用连接酶检测反应(LDR)检测患者基因类型,分析TNIP1基因多态性与LN易感性、临床特征、免疫制剂疗效及感染风险的关系。结果LN组和对照组TNIP1基因多态性rs7708392位点的基因型和等位基因分布频率比较,差异具有统计学意义(P<0.05)。不同病理分型、是否有血液系统损伤、抗ds-DNA是否阳性、抗心磷脂抗体是否阳性的患者rs7708392位点的基因型分布比较,差异具有统计学意义(P<0.05);是否有血液系统损伤患者rs10036748位点的基因型分布比较,差异具有统计学意义(P<0.05)。不同治疗效果、感染情况的LN患者rs7708392位点的基因型和等位基因分布频率比较,差异具有统计学意义(P<0.05)。结论TNIP1基因rs7708392位点多态性可能参与狼疮性肾炎的发生发展,与其易感性、临床表现、免疫制剂疗效及感染风险密切相关。OBJECTIVE To explore the relationship between gene polymorphisms of human tumor necrosis factorα-induced protein 3(TNFAIP3)interaction protein 1(TNIP1)and lupus nephritis(LN).METHODS A total of 105 patients confirmed with LN by renal biopsy in Nephrology Department of Anqing First People’s Hospital from Jan 2017 to Mar 2019 were enrolled.The other 56 normal subjects undergoing physical examination during the same period were enrolled as the control group.5 mL elbow venous blood was collected from all the subjects.The single nucleotide polymorphisms(SNPs)from the three loci(rs7708392,rs10036748,rs13168551)in TNIP1 gene region was detected.Genotypes were detected by ligase detection reaction(LDR).The relationship between TNIP1 gene polymorphism and LN susceptibility,curative effect of immune preparations and infection risk was analyzed.RESULTS The differences in genotypes and alleles distribution frequencies of TNIP1 gene polymorphism at rs7708392 locus were significant between the LN group and control group(P<0.05).The differences in genotype distributions at rs7708392 loci were significant among patients with different pathological types,blood system damage,positive anti-ds-DNA and positive anti-cardiolipin antibodies(P<0.05).The differences in genotype distributions at rs10036748 loci were significant among patients regardless of blood system damage(P<0.05).The differences in genotypes and alleles distribution frequencies at rs7708392 locus were significant among LN patients with different curative effect and infection status(P<0.05).CONCLUSION The polymorphism of TNIP1 gene at rs7708392 locus may be involved in the occurrence and development of LN,which is closely related to its susceptibility,clinical manifestations,curative effect of immune preparations and infection risk.
