厚朴排气合剂联合针刺穴位治疗胸腰椎术后腹胀临床观察

摘　　要：目的探讨厚朴排气合剂联合针刺穴位治疗胸腰椎术后腹胀的效果,以及对患者胃肠功能的影响。方法选取医院2018年2月至2020年2月收治行胸腰椎内固定术后伴腹胀的患者112例,随机分为观察组和对照组,各56例。两组患者均针刺足三里穴、合谷穴、内关穴,观察组患者加服厚朴排气合剂。结果观察组中1例自愿退出研究、2例未严格执行治疗方案,对照组中2例因病情变化而调整治疗方案、2例研究数据缺失不完整,均予以剔除。观察组总有效率为90.57%,明显高于对照组的73.08%(P<0.05);与对照组比较,治疗后观察组患者肠鸣音恢复、首次排气、首次排便、腹胀消失时间均明显缩短,胃动素、胃泌素、胃肠P物质水平均明显升高,C反应蛋白、皮质醇、去甲肾上腺素水平均明显降低(P<0.05);观察组与对照组不良反应发生率相当(16.98%比9.62%,P>0.05)。结论厚朴排气合剂联合针刺穴位治疗胸腰椎术后腹胀有一定疗效,能促进患者胃肠道功能的恢复,改善腹胀症状,升高胃肠道功能指标水平,减轻机体的氧化应激状态。Objective To investigate the efficacy of Houpupaiqi Mixture combined with acupoint acupuncture in the treatment of patients with abdominal distension after thoracolumbar surgery and its effect on gastrointestinal function.Methods A total of 112 patients with abdominal distension after thoracolumbar surgery admitted to our hospital from February 2018 to February 2020 were selected and randomly divided into the observation group(56 cases)and the control group(56 cases).The patients in the two groups were acupunctured at Zusanli point,Hegu point and Neiguan point,and the patients in the observation group were treated with Houpupaiqi Mixture.Results In the observation group,one patient voluntarily withdrew from the study,two patients did not strictly implement the treatment plan.In the control group,two patients adjusted the treatment plan due to the change of disease condition,and two patients with incomplete studying data were excluded from the study.The total effective rate of the observation group was 90.57%,which was significantly higher than 73.08%of the control group(P<0.05).After treatment,the bowel sounds recovery time,first exhaust time,first defecation time and abdominal distension disappearance time in the observation group were significantly shorter than those in the control group(P<0.05).The levels of motilin,gastrin and gastrointestinal substance P in the observation group were significantly higher than those in the control group,while the levels of C-reactive protein(CRP),cortisol(Cor)and norepinephrine(NE)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(16.98%vs.9.62%,P>0.05).Conclusion Houpupaiqi Mixture combined with acupoint acupuncture has a certain effect in the treatment of patients with abdominal distension after thoracolumbar surgery.It can promote the recovery of gastrointestinal function,improve the symptoms of abdominal distention,increase the levels
