韩明向辨治咳嗽的临证策略探析

摘　　要：以咳嗽的辨证策略和治疗策略为切入点,总结韩明向教授辨治咳嗽的临床经验。韩教授根据咳嗽的发病特点、证候特征、转归以及预后等,归纳总结出了咳嗽"四依合参"的辨证策略,即"依起病辨内外、依咳痰辨病性、依邪正辨虚实、依脏腑辨病位"。同时韩教授提出了六点临床治疗咳嗽的策略,即以止嗽散为基础方,灵活加减;首宣肺宣肃并用;治咳不忘利肺窍;肺喜温常用温肺;肺热证重用清肺;补肺气兼补脾肾。另外,韩教授亦十分强调咳嗽用药之"治咳嗽常忌六太"的禁忌理论以及患者预防调护的重要性,以达到标本兼顾、防治结合的目的。Based on the dialectical and therapeutic strategies of cough, this paper summarizes Professor Han Mingxiang’s clinical experience in the treatment of cough. According to the characteristics of cough, syndrome, outcome and recovery, Professor Han summed up the dialectical strategy of "four principles of combination and reference", that is, "differentiation of internal and external according to the onset of disease, differentiation of disease nature according to phlegm and saliva, differentiation of deficiency and actual according to pathogenic factors and differentiation of disease position according to viscera". At the same time, Professor Han put forward six strategies for clinical treatment of cough, that is, based on Zhisou powder, flexible addition and subtraction;the first Xuanfei Xuansu combined;the treatment of cough does not forget to benefit the lung orifices;the lung likes to warm commonly used warm the lung;lung heat syndrome reuse Qingfei;lung qi and spleen and kidney. In addition, Professor Han also stressed the taboo theory of "six taboos for cough treatment" and the importance of prevention and care of patients, so as to achieve the purpose of combining prevention and treatment.
