甘花漱口液联合喉风散局部喷药治疗头颈部肿瘤急性放射性咽炎的临床疗效观察

摘　　要：目的探讨甘花漱口液联合喉风散局部喷药治疗头颈部肿瘤急性放射性咽炎的临床疗效。方法选取我院于2015年1月—2020年12月收治的60例头颈部肿瘤急性放射性咽炎患者,随机分为实验组和对照组各30例,对照组给予喉风散局部喷药治疗,实验组给予院内制剂甘花漱口液联合喉风散局部喷药治疗,对比分析两组患者治疗前及治疗2周后的中医症状学积分、血清炎症因子[C-反应蛋白(CRP)、肿瘤坏死因子(TNF-α)、降钙素原(PCT)、白细胞介素-6(IL-6)]、免疫功能指标[血液CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)、血管内皮黏附因子(VACM-1)、唾液免疫球蛋白A(SIgA)]水平变化情况;比较治疗2周后两组患者口咽黏膜损伤和疼痛程度差异。结果治疗2周后对比治疗前,两组患者各项中医症状积分、CRP、TNF-α、PCT、IL-6、CD_(8)^(+)均降低,实验组与同一时期对照组相比显著更低(P<0.05)。两组CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、VACM-1、SIgA均升高,且差异均有统计学意义(P<0.05),实验组与同一时期对照组相比显著更高(P<0.05)。治疗2周后,实验组口咽黏膜损伤和疼痛程度较对照组显著更低(P<0.05)。结论院内制剂甘花漱口液联合喉风散局部喷药可改善机体免疫功能,减轻炎症反应,更有利于缓解放射所致的黏膜损伤,减轻疼痛,能有效改善头颈部肿瘤急性放射性咽炎患者疾病预后。Objective To investigate the clinical efficacy of hospital preparation Ganhua mouthwash combined with local spray of Houfeng powder in the treatment of head and neck tumors with acute radiation pharyngitis.Methods 60 patients with head and neck tumors complicated with acute radiation pharyngitis admitted to the hospital were selected between January 2015 and December 2020, and they were randomly divided into experimental group and control group with 30 cases in each group. The control group was treated with local spray of Houfeng powder, and the experimental group was given hospital preparation Ganhua mouthwash combined with Houfeng powder local spray. The scores of TCM symptoms, serum inflammatory factors [C-reactive protein(CRP), tumor necrosis factor(TNF-α), procalcitonin(PCT), interleukin-6(IL-6)], immune function indicators [blood CD_(4)^(+), CD_(8)^(+), CD_(4)^(+)/CD_(8)^(+), vascular cell adhesion molecule(VACM-1), salivary immunoglobulin A(SIgA)] were compared between the two groups of patients before treatment and after 2 weeks of treatment. Differences in oropharyngeal mucosal injury degree and pain degree of the two groups were compared after 2 weeks of treatment.Results After 2 weeks of treatment, the scores of TCM symptoms and levels of CRP, TNF-α, PCT, IL-6 and CD_(8)^(+) of the two groups of patients were reduced compared to before treatment, and the scores and levels of experimental group were significantly lower than those of control group during the same period(P<0.05). The levels of CD_(4)^(+), CD_(4)^(+)/CD_(8)^(+), VACM-1 and SIgA were all increased in the two groups(P<0.05), and the levels of experimental group were significantly higher compared with those of control group during the same period(P<0.05). After 2 weeks of treatment, the oropharyngeal mucosal injury degree and pain degree in experimental group were significantly milder than those in control group(P<0.05).Conclusion Hospital preparation Ganhua mouthwash combined with local spray of Houfeng powder can improve the body’ s imm
