摘 要:目的研究T-box转录因子21(TBX21)rs11657479多态性与缺血性中风(IS)中医证候及其血脂水平的关联。方法纳入病例组及对照组各774例、793例。中风患者的辨证分型采用《中风病辨证诊断标准(试行)》标准。运用Massarray技术对TBX21基因多态性位点rs11657479进行基因分型,运用PLINK软件分析遗传关联数据。结果(1)TBX21基因rs11657479位点的基因型频率在缺血性中风火热证组与对照组之间的分布差异明显,具有统计学意义(P=0.002)。(2)TBX21基因rs11657479多态性与缺血性中风的发生风险无显著关联(各遗传模型均P>0.05)。(3)校正性别、年龄后,TBX21基因rs11657479位点多态性仍与缺血性中风火热证的发生风险显著相关[隐性模型:OR_(adj)=17.35,95%CI_(adj)(1.54-196.10),P_(adj)=0.021];未发现rs11657479多态性与缺血性中风其它证候的发生风险显著相关(均P>0.05)。(4)校正性别、年龄后,TBX21基因rs11657479多态性与缺血性中风患者的APO-A1水平[加性模型:β(95%CI)_(adj)=0.08(0.01-0.15),P_(adj)=0.021;显性模型:β(95%CI)_(adj)=0.09(0.01-0.16),P_(adj)=0.021]、HDL-C水平[加性模型:β(95%CI)_(adj)=0.08(0.02-0.15),P_(adj)=0.013;显性模型:β(95%CI)adj=0.08(0.01-0.16),P_(adj)=0.022]相关联。结论TBX21基因rs11657479多态性可能影响缺血性中风火热证的发生。Objective To study the association of T-box transcription factor 21(TBX21) rs11657479 polymorphism with TCM syndromes of ischemic stroke(IS) and their blood lipid levels. Methods We enrolled 774 patients with ischemic stroke, 793 healthy patients and orthopedic patients with mild trauma. According to Diagnostic Criteria for Stroke Syndrome Differentiation(Trial), 774 patients with IS were included in TCM syndrome differentiation. Massarray technology was used to genotype the TBX21 gene polymorphism site rs11657479, and PLINK statistical software was used for genetic association analysis. Results(1)The genotype frequency of the rs11657479 of the TBX21 gene was statistically significant between heat syndrome group of ischemic stroke and the control group(P=0.002).(2)The rs11657479 polymorphism of TBX21 gene was not significantly associated with the risk of ischemic stroke(all genetic models P>0.05).(3)After adjusting for gender and age, the association between the rs11657479 polymorphism and the risk of ischemic stroke-heat syndrome was statistically significant[recessive model: OR_(adj)=17.35, 95%CIadj(1.54-196.10), P_(adj)=0.021]. No significantly associated relation was found between rs11657479 polymorphism and the risk and other TCM syndromes of ischemic stroke(both P>0.05).(4)After adjusting for gender and age, the correlation of TBX21 gene rs11657479 polymorphism and APO-A1 level[additive model: β(95%CI)adj=0.08(0.01-0.15),P_(adj)=0.021;dominant model: β(95%CI)adj=0.09(0.01-0.16),P_(adj)=0.021] and HDL-C level[additive model: β(95%CI)adj=0.08(0.02-0.15), P_(adj)=0.013;dominant model: β(95%CI)adj=0.08(0.01-0.16), P_(adj)=0.022] was statistically significant. Conclusion The rs11657479 polymorphism of TBX21 gene may affect the occurrence of ischemic stroke of heat syndrome.
