摘 要:目的探讨腹型过敏性紫癜患儿血清干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、肠型脂肪酸结合蛋白(I-FABP)水平及临床意义。方法选取2018年1月至2020年9月就诊于该院的腹型过敏性紫癜患儿122例作为观察组,根据有无消化道出血症状分为消化道出血组43例,消化道无出血组79例。另选取同期于该院进行常规体检的健康儿童122例作为对照组。酶联免疫吸附试验(ELISA)法检测各组血清IFN-γ、IL-4、I-FABP水平,采用Pearson相关分析血清IFN-γ、IL-4、I-FABP之间的相关性,采用受试者工作特征曲线(ROC曲线)分析血清IFN-γ、IL-4、I-FABP单独检测及三者联合对腹型过敏性紫癜的诊断价值。结果观察组血清IFN-γ水平较对照组低,IL-4、I-FABP水平均较对照组高,差异均有统计学意义(P<0.05);消化道出血组IFN-γ水平较消化道无出血组低,IL-4、I-FABP水平均较消化道无出血组高,差异均有统计学意义(P<0.05);Pearson分析结果显示,观察组血清IFN-γ与IL-4、I-FABP均呈负相关(r=-0.618、-0.531,P<0.05),IL-4与I-FABP呈正相关(r=0.605,P<0.05);ROC曲线分析结果显示,血清IFN-γ、IL-4、I-FABP单独检测腹型过敏性紫癜的曲线下面积(AUC)依次为0.786、0.829、0.810,三者联合检测腹型过敏性紫癜AUC为0.937,显著高于三者单独检测(P<0.05)。结论腹型过敏性紫癜患儿血清中IFN-γ低表达,IL-4、I-FABP高表达,三者联合检测对腹型过敏性紫癜具有较高诊断价值,可能为临床上腹型过敏性紫癜的早期诊断提供参考。Objective To explore the levels and clinical significance of interferon-γ(IFN-γ),interleukin-4(IL-4),intestinal fatty acid binding protein(I-FABP)in serum of children with abdominal anaphylactoid purpura.Methods 122 children with abdominal anaphylactoid purpura who were treated in a hospital from January 2018 to September 2020 were selected as the observation group,which was divided into 43 patients in gastrointestinal hemorrhage group and 79 patients in gastrointestinal non hemorrhage group according to the symptoms of gastrointestinal bleeding.In addition,122 healthy children who had routine physical examination in our hospital at the same time were selected as the control group.The levels of IFN-γ,IL-4 and I-FABP in serum of each group were detected by enzyme-linked immunosorbent assay(ELISA).Pearson analysis of serum IFN-γ,IL-4,I-FABP correlation.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of the levels of serum IFN-γ,IL-4 and I-FABP,and the combination of the three in abdominal allergic purpura.Results The level of IFN-γin the serum of the observation group was lower than that of the control group,and the levels of IL-4 and I-FABP were higher than that of the control group,the difference was statistically significant(P<0.05);The level of IFN-γin gastrointestinal hemorrhage group was lower than that in gastrointestinal non hemorrhage group,and the levels of IL-4 and I-FABP were higher than those in gastrointestinal non hemorrhage group,the difference was statistically significant(P<0.05);Pearson correlation analysis showed that IFN-γin serum of observation group was negatively correlated with IL-4 and I-FABP(r=-0.618,-0.531,P<0.05),while IL-4 was positively correlated with I-FABP(r=0.605,P<0.05);ROC curve results showed that area under the curve(AUC)of IFN-γ,IL-4,I-FABP in serum detecting abdominal anaphylactoid purpura was 0.786,0.829,0.810,and the AUC of the three combination was 0.937,which was significantly higher than that of the three single detections(P
