腹腔镜与开腹阑尾切除术在急性穿孔性阑尾炎治疗中的疗效分析

摘　　要：目的探讨腹腔镜与开腹阑尾切除术在急性穿孔性阑尾炎治疗中的疗效。方法选取2019年1月至2020年1月本院收治的100例急性穿孔阑尾炎患者,以入院编号单双数分为常规组与观察组,每组50例。常规组采用开腹阑尾切除术治疗,观察组采用腹腔镜阑尾切除术治疗,比较两组围术期相关指标、血清炎症相关指标及术后并发症发生率。结果观察组术中出血量少于常规组,且术后各项时间指标均短于常规组,差异有统计学意义(P<0.05);术后两组血清炎症指标均上升,但观察组均低于常规组(P<0.05);治疗后,观察组并发症发生率低于常规组(P<0.05)。结论使用腹腔镜阑尾切除术治疗急性穿孔性阑尾炎患者,术后恢复时间更短,且应激反应和并发症发生率较低,安全性较高,值得临床推广。Objective To investigate the efficacy of laparoscopic and open appendectomy in the treatment of acute perforated appendicitis.Methods 100 patients with acute perforated appendicitis who were admitted to our hospital from January 2019 to January 2020 were selected and they were divided into the routine group and the observation group by single and even numbers,with 50 cases in each group.The routine group was treated with open appendectomy,while the observation group was treated with laparoscopic appendectomy.The perioperative indicators,serum inflammatory indicators and postoperative complication rate between the two groups were compared.Results The intraoperative blood loss in the observation group was less than that in the routine group,and the postoperative time indexes were shorter than those in the routine group,with statistically significant differdence(P<0.05).After operation,serum inflammatory indexes in both groups were increased,but the observation group were lower than the routine group(P<0.05).After treatment,the incidence of complications in the observation group was lower than that in the routine group(P<0.05).Conclusion Laparoscopic appendectomy for patients with acute perforated appendicitis has shorter postoperative recovery time,lower incidence of stress response and complications,higher safety,and it is worthy of clinical promotion.
