血管内支架置入术治疗非血栓性上矢状窦狭窄的临床观察

摘　　要：目的评估上矢状窦(SSS)支架置入术治疗非血栓性颅内静脉闭塞性疾病(VOD)的安全性和有效性。方法回顾性分析2016年2月至2019年10月本院收治的19例因症状性上矢状窦狭窄而行SSS支架置入术患者的临床资料。所有患者术前均经DSA明确诊断并采用静脉窦内测压。收集所有患者的人口学资料及临床资料。均采用血管内支架置入术治疗,并通过临床随访观察其治疗效果。结果11例(57.9%)为原发性SSS狭窄,8例(42%)为横窦支架置入后继发性SSS狭窄。支架植入后SSS的最大平均静脉压(MVP)由16.2 mmHg降至13.1 mmHg(P=0.037)。狭窄两侧的压力梯度由4.2 mmHg降至1.5 mmHg(P<0.001)。未见术中并发症。术后随访平均(6.2±2.3)个月,未见术后支架内再狭窄及血栓形成。结论SSS支架置入术安全性高,可改善临床症状,值得临床推广应用。Objective To evaluate the safety and efficacy of superior sagittal sinus(SSS)stenting in the treatment of non-thrombotic intracranial vein occlusion disease(VOD).Methods From February 2016 to October 2019,19 patients who received SSS stent implantation due to symptomatic superior sagittal sinus stenosis were retrospectively analyzed.All preoperative patients were diagnosed by DSA and intravenous sinus pressure measurement.General,demographic and clinical data of all patients were collected.All patients were treated with endovascular stent implantation,and the therapeutic effect was observed through clinical follow-up.Results 11 cases(57.9%)were primary SSS stenosis,and 8 cases(42%)were secondary SSS stenosis after transverse sinus stent implantation.The maximum mean venous pressure(MVP)of SSS decreased from 16.2 mmHg to13.1 mmHg(P=0.037);after stent implantation,the pressure gradient on both sides of the stenosis decreased from 4.2 mmHg to 1.5 mmHg(P<0.001).No intraoperative complications were observed.The mean postoperative follow-up was(6.2±2.3)months,and no postoperative stent restenosis and thrombosis were observed.Conclusion SSS stent implantation is safe,can improve clinical symptoms,which is worthy of clinical promotion and application.
