居家肝移植受者睡眠质量现状及其影响因素分析

摘　　要：目的调查肝移植受者居家期间睡眠质量现状,并进一步探讨其影响因素。方法选取2019年7月至2020年1月在中山大学附属第一医院器官移植中心术后复诊的107例肝移植受者,使用一般资料问卷和匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)量表进行调查。不符合正态分布的计量资料以中位数(四分位数)表示,采用秩和检验比较。采用多元线性回归分析居家肝移植受者PSQI量表总分的影响因素。结果肝移植患者PSQI总分中位数(四分位数)为7(5,10)分。43%的受者PSQI量表总分>7分,睡眠质量较差。单因素分析结果显示,年龄、工作状态和术后时间是居家肝移植受者睡眠质量的影响因素(P均<0.05)。多因素分析结果显示,术后时间是居家肝移植受者PSQI量表总分的影响因素(P<0.05)。结论居家肝移植受者睡眠质量较差,其中以入睡困难、睡眠障碍居多。年龄大、不在岗、术后时间≤2年的肝移植受者睡眠质量有待提高,医护人员应加强关注此类受者的睡眠状况,改善及提高其睡眠质量及生存质量。Objective To investigate the status of sleep quality of liver transplant recipients at home and to explore the influencing factors.Methods A total of 107 liver transplant recipients were selected from July 2019 to January 2020 in the Department of Organ Transplantation,the First Affiliated Hospital of Sun Yat sen University.The general information questionnaire and Pittsburgh Sleep Quality Index(PSQI)were used to investigate.Results The total PSQI score of liver transplant recipients at home was[7(5,10)].About 43%of patients with PSQI>7 had poor sleep quality.Age,working state and duration of liver transplantation were the influencing factors of sleep quality at home(P<0.05).Conclusions The sleep quality of liver transplant recipients at home is lower than that of normal adults.The sleep quality of liver transplant recipients who are older,not on the job,and≤2 years of transplantation need to be improved.Medical staff should pay attention to the sleep care of these patients and improve their sleep quality and quality of life.
