院前救治的儿童意识障碍浅析

摘　　要：目的探讨分析院前救治意识障碍儿童的病因特点。方法采用回顾性分析方法,对2019-01至2019-12月期间由镇江市急救中心救治的格拉斯哥昏迷评分(Glasgow Coma Scale,GCS)≤9且年龄≤14岁患者数据进行回顾性分析。结果共有133名儿童(占当年度救治患者总数的0.83%)被纳入研究,其中57.9%为男性。年龄1-4岁者占48.1%。抽搐是呼叫院前急救的最常见原因,共54例(40.6%),最常见于学龄前儿童组,与其他年龄组别中有明显差异(P<0.001)。监测指标(心率、血氧饱和度、GCS)从现场初次评估到抵达医院期间改善或保持稳定。结论抽搐发作是院前急救中儿童意识障碍最常见的原因,并且最常见于学龄前儿童。在院前医护人员的救治期间,儿童患者的监测指标和意识障碍得到一定程度改善。Objective To investigate the etiological characteristics of children with disturbance of consciousness before hospitalization.Methods Retrospective analysis was carried out on the patients,whose Glasgow Coma Score(GCS)≤9 and age≤14 years old,that were admitted to Zhenjiang Emergency Center from January 2019 to December 2019.Results A total of 133 children(accounting for 0.83%of the total number of patients treated in that year)were included in the study,of which 57.9%were males.Patients aged from 1 to 4 years old accounted for 48.1%.Convulsion was the most common cause of calling pre-hospital first aid,with a total of 54 cases(40.6%),and most seen in the pre-school age group with significant difference(P<0.001).Vital signs(heart rate,blood oxygen saturation,GCS)improved or remained stable from initial evaluation at the scene to arrival at the hospital.Conclusion Convulsion is the most common cause of calling pre-hospital first aid in children with changed consciousness and is most seen in pre-school children.During the prehospital treatment,the vital signs and consciousness levels of pediatric patients were improved to a certain extent.
